摘要
目的探讨宫腔镜及经阴道超声在老年女性子宫内膜病变的临床价值应用。方法回顾性分析2015年1月—2018年8月我院收治的疑似子宫内膜病变的老年患者110例的临床资料,均行经阴道超声及宫腔镜检查,将病理检查作为对照,以此分析宫腔镜及经阴道超声超检查的临床价值。结果经阴道超声共提示异常病例104例,经病理证实102例;正常病例6例,经病理证实5例。经阴道超声检查敏感度为99.03%(102/103),特异度71.43%(5/7),阳性预测值98.08%(102/104),阴性预测值83.33%(5/6)。宫腔镜诊断结果与病理检查对比发现,子宫内膜息肉、萎缩性子宫内膜、子宫粘膜下肌瘤、子宫内膜增生、子宫内膜炎、子宫内膜癌等检出率无明显差异,对比无统计学意义(P>0.05)。结论宫腔镜及经阴道超声检查对诊断老年女性子宫内膜病变有重要作用,经阴道超声可用于初筛,宫腔镜对于明确病因具有显著优势。
Objective To explore the clinical application of hysteroscopy and transvaginal ultrasound in elderly women with endometrial lesions.Methods A retrospective analysis was made to the clinical data of 110 elderly women with suspected endometrial lesions hospitalized during the period from Jan.,2015 to Aug.,2018;examinations by transvaginal ultrasound and hysteroscopy were performed to all the cases and contrast pathological studies were made.Results104cases were revealed abnormal and6cases normal by transvaginal ultrasound while102cases out of the104and5cases out of the6were confirmed by pathological study;the sensitivity of transvaginal ultrasound was 99.03%(102/103),the specificity 71.43%(5/7),the positive predictive value 98.08%(102/104)and the negative predictive value 83.33%(5/6);the contrast study between hysteroscopy examination and pathological study indicated that there existed no statistical difference in detection rate of endometrial polyps,atrophic endometrium,submucosalmyoma,endometrial hyperplasia,endometritis and endometrial carcinoma(P>0.05).Conclusions Hysteroscopy and transvaginal ultrasound play an important role in the diagnosis of endometrial lesions in elderly women;transvaginal ultrasound may be applied in primary screening and hysteroscopy is of obvious advantage in determining the causes.
