摘要
目的分析妊娠早期肝肾代谢指标水平与妊娠期糖尿病(GDM)的相关性。方法回顾性分析2016年1月至2018年3月在我院确诊为GDM的344例孕妇作为观察组,并随机选取同期糖耐量实验正常的336例孕妇作为对照组,比较分析两组孕妇的一般资料、肝肾功能指标水平,采用Logistic回归分析GDM发生的危险因素。结果与对照组孕妇相比,观察组孕妇血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、γ-谷氨酰转肽酶(GGT)、尿素氮(BUN)、尿酸(UA)、肌酐(Cre)及胱抑素C(Cys-C)水平明显升高(P<0.05),而总胆红素(TBIL)、直接胆红素(DBIL)、白蛋白(Alb)和胆碱酯酶(CHE)水平明显降低(P<0.05); Logistic回归结果显示孕前BMI、孕次、ALT、AST、TGT、BUN、UA及Cys-C是以后发生GDM的独立危险因素,而TBIL、DBIL及Alb则为其保护因素。结论孕前BMI、孕次、ALT、AST、GGT、BUN、UA、Cys-C是以后发生GDM的独立危险因素,且TBIL、DBIL和Alb处于低水平时也是以后发生GDM的危险因素,应积极检测并预防,以降低GDM的发病风险。
Objective To analyze the relationship between gestational diabetes mellitus(GDM)and hepatic and renal metabolism levels in early pregnancy.Methods From Jan 2016 to Mar 2018,344 patients with GDM were retrospectively analyzed in our hospital,and 336 normal glucose tolerance pregnant women were randomly chosen as the control.The general data,hepatic and renal function parameters of two groups were compared and analyzed.Risk factors were analyzed by using the logistic regression analysis.Results Compared with those of the control group,serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma-glutamyl transferase(GGT),urea nitrogen(BUN),uric acid(UA),creatinine(Cre)and Cystine C(Cys-C)levels were significantly increased in the observed group,but the total bilirubin(TBIL),direct bilirubin(DBIL),albumin(Alb)and cholinesterase(CHE)were significantly decreased.BMI value,the number of pregnancy,ALT,AST,GGT,BUN,UA and Cys-C levels were the independent risk factors of GDM,however,TBIL,DBIL and Alb were the protective factors.Conclusion BMI,ALT,AST,GGT,BUN,UA,and Cys-C levels are risk factors of GDM.Also the low levels of TBIL,DBIL,and Alb are risk factors of GDM.Active detection of these indexes and early prevention will help reduce the risk for the occurrence of GDM.
