摘要
目的采用静息态功能磁共振技术研究有流涎症状的帕金森患者大脑皮层-纹状体回路神经连接情况。方法本实验招募了34例未经药物治疗的帕金森早期患者,其中17例有流涎症状为流涎组,17例无流涎症状为非流涎组。另招募34例性别、年龄匹配的健康人作为正常对照组。使用1. 5T MR仪器采集所有参与者静息态功能磁共振数据,统计分析患者纹状体的功能连接情况。结果帕金森流涎组的壳核与双侧感觉运动皮层、顶上回、顶下回以及右颞、枕叶脑区的功能连接明显低于非流涎组(所有P <0. 05)。与健康对照组相比,患者大脑皮层-纹状体回路功能连接异常。壳核后部功能连接显著减弱,纹状体、尾状核后部功能连接增强(所有P <0. 05)。结论本研究发现帕金森流涎患者的静息态功能连接异常,尤其是皮层-纹状体网络功能连接异常。本实验将有助于推动对帕金森患者流涎潜在神经机制的进一步研究。
Objective Drooling is one of the main symptoms of patients with Parkinson disease (PD). We evaluated the brain function of striatal circuit in PD patients with drooling by using resting-state functional magnetic resonance. Methods We recruited 34 early-stage, treatment-naive PD patients, 17 cases of whom with drooling were classified as drooling group, the other 17 people without drooling as non-drooling group. A total of 34 sex, age-matched healthy individuals were recruited as normal controls. The resting state fMRI data of all participants were acquired on a 1.5TMR system, and the functional connectivity of the corpus striatum was analyzed. Results This study found significantly lower functional connetivity between putamen with bilatral sensorimotor cortices, superior and inferior parietal gyrus, right occipital and temporal gyrus than PD patiens without drooling (All P <0.05). Compared with the healthy control group, both of the PD subgroups showed aberrant functional connectivity in cortico-striatal circuitry. The functional connectivity of the posterior putamen decreased significantly, and that of the striatum and posterior caudate enhanced significantly (all P <0.05). Conclusion This study found abnormalities of neural functional connectivity, especailly within the cortico-striatal circuitry, in early-stage, treatment-na ve PD patients with drooling. This study might be helpful for the further study of the underlying neural mechanisms of drooling symptom in PD patients.
