摘要
目的:探讨临床药师干预对高血压病患者疗效及用药依从性的影响。方法:将120例血压控制不佳的原发性高血压病患者随机分为对照组和临床药师干预组,每组60例。对照组患者由医院药师建立健康档案并定期随访,提供高血压病相关知识教育、用药指导及药物咨询等药学服务;临床药师干预组患者由临床药师采用药物治疗管理模式对患者提供药学监护。以用药依从性优占比、血压达标率及生活质量评分为指标,评估两组患者的干预效果。结果:干预12个月后,观察组患者的用药依从性优占比、血压控制率及平均生活质量评分分别为86.7%(52/60)、90.0%(54/60)及(578.56±118.20)分,对照组患者分别为73.3%(44/60)、73.3%(44/60)及(532.10±132.25)分,两组患者各项指标水平较干预前均有明显改善,且临床药师干预组患者明显优于对照组,差异均有统计学意义(P<0.05)。结论:药师干预可有效提高高血压病患者的疗效及用药依从性,药物治疗管理模式尤其能体现药师专业价值,对高血压病管理有重要价值。
OBJECTIVE: To probe into the effects of intervention on the efficacy and medication compliance in patients with hypertension by clinical pharmacists. METHODS: 120 patients with primary hypertension with poor blood pressure control were randomly divided into control group and intervention group, with 60 cases in each group. Pharmacists had established health records and conducted regular follow-up for patients in control group, and provided pharmaceutical service such as disease education on hypertension, medication guidance and pharmaceutical counseling; the intervention group was given pharmaceutical care by clinical pharmacists using the drug treatment management model. The intervening effects were evaluated by the superior ratios of medication compliance, control rates of blood pressure and scores of life quality. RESULTS: After 12-month intervention, the superior ratio of medication compliance, control rate of blood pressure and average score of life quality of the observation group were respectively 86.7%(52/60), 90.0%(54/60) and(578.56±118.20) points, while those of the control group were respectively 73.3%(44/60), 73.3%(44/60) and(532.10±132.25) points, all indices had been significantly improved compared with those before intervention, and those of the intervention group were significantly better than the control group, with statistically significant differences(P<0.05). CONCLUSIONS: Intervention by pharmacist can effectively improve the efficacy and medication compliance of patients with hypertension, the drug treatment management model can especially reflect the professional value of pharmacists and is of great value to the management of hypertension.
