摘要
目的探讨局部麻醉和硬膜外麻醉应用于腹股沟疝无张力修补术的疗效。方法随机选取2017年1月—2018年1月间该院收治的90例腹股沟疝症患者为研究对象,按照随机数字表法分为局部麻醉组和硬膜外麻醉组各45例,分析反映两组患者治疗效果指标之间的差异是否具有统计学意义。结果局部麻醉组患者手术时间为(40.53±3.11)min,术后下床活动时间为(4.16±1.14)h,3例患者术中出现不良反应(发生率为6.67%),8例患者出现术后切口疼痛(发生率为17.78%),9例患者出现术后并发症(发生率为20.00%);硬膜外麻醉组患者的手术时间为(53.82±3.65)min,术后下床活动时间为(12.33±1.36)h,10例患者术中出现不良反应(发生率为22.22%),19例患者出现术后切口疼痛(发生率为42.22%),18例患者出现术后并发症(发生率为40.00%),两组患者手术时间、术后下床活动时间以及术中不良反应、术后切口疼痛和并发症发生率之间差异有统计学意义(P<0.05)。结论局部麻醉应用于腹股沟疝无张力修补术的疗效优于硬膜外麻醉,具有手术时间和术后下床活动时间短、舒适度和手术满意度高、安全有效的特点,值得推广应用。
Objective To study the curative effect of different anesthesia methods in the tension-free inguinal hernia repair.Methods 90 cases of patients with inguinal hernia admitted and treated in our hospital from January 2017 to January 2018 were randomly selected and randomly divided into two groups with 51 cases in each,and the difference in the treatment effect between groups was statistically significant.Results The operation time and postoperative time to get out of bed in the local anesthesia group were respectively(40.53±3.11)min and(4.16±1.14)h,and 3 cases had the adverse reactions,and the incidence rate was 6.67%,and 8 cases had the postoperative incision pains,and the incidence rate was 17.78%,and 9 cases had the postoperative complications,and the incidence rate was 20.00%,and the operation time,time to get out of bed after surgery in the epidural anesthesia group were respectively(53.82±3.65)min and(12.33±1.36)h,and 10 cases had the advese reactions,and the incidence rate was 22.22%,19 cases had the postoperative incision pains,and the incidence rate was 42.22%,18 cases had the postoperative complications,and the incidence rate was 40.00%,and the differences in the operation time,time to get out of bed,intraoperative adverse reactions,postoperative incision pains and incidence rate of complications between the two groups were statistically significant(P<0.05).Conclusion The curative effect of local anesthesia in the tension-free inguinal hernia repair is better than that of epidural anesthesia,with the short operation time and time to get out of bed,and high comfort degree,operative satisfactory degree,and it is safe and effective,and it is worth promotion and application.
