摘要
目的:探讨超声在消化道穿孔患者中的诊断效果及影像特点。方法:选择消化道穿孔患者50例,入选患者均经过手术病理检查得到确诊,入院后患者先行X线检查,对于疑似病灶再行超声检查,将两种诊断结果与手术病理结果进行比较,分析超声在消化道穿孔患者中的诊断效果及影像特点。结果:50例消化道穿孔患者均经过手术检查得到确诊,患者行X线检查确诊39例,确诊率为78. 00%,超声诊断确诊率为94. 00%,超声诊断与手术诊断比较,差异无统计学意义(P> 0. 05),但高于X线诊断,差异有统计学意义(P <0. 05); 20例腹腔游离气体超声下可见肝前存在不同程度的强回声干扰,存在多次发射回声。11例腹腔积液超声下显示:大量浑浊的液体,左肝前可见游动气体,右上腹可见不均质包块。结论:消化道穿孔患者入院后采用超声检查能帮助患者确诊,诊断符合率较高,能弥补X线的不足,为临床治疗提供依据和参考,值得推广应用。
Objective To investigate the diagnostic and imaging features of ultrasound in perforated patients with digestive tract.Method Fifty patients with perforation of gastrointestinal tract were enrolled.All patients were confirmed by surgical pathological examination.X-ray examination was performed after admission.For the suspected lesions,ultrasound examination was performed.Compared with the surgical pathological results,the diagnostic and imaging features of ultrasound in perforation of digestive tract were analyzed.Results 50 cases of perforation of gastrointestinal tract were confirmed by surgical examination.The patients underwent X-ray examination and confirmed 39 cases.The diagnosis rate was 78.00%,and the diagnosis rate of ultrasound diagnosis was 94.00%.There was no significant difference between ultrasound diagnosis and surgical diagnosis(P>0.05),but higher than the X-ray diagnosis(P<0.05);20 cases of intraperitoneal free gas ultrasound showed different levels of strong echo before the liver,there were multiple echoes emitted.Eleven cases of abdominal effusion under ultrasound showed:a large amount of turbid fluid,visible swimming gas in front of the left liver,and heterogeneous masses in the right upper quadrant.Conclusion Ultrasound examination can help the patient diagnosed after admission to the digestive tract perforation.The rate of diagnosis is high,which can make up for the lack of X-ray.It provides a basis and reference for clinical treatment and is worthy of popularization and application.
