左甲状腺素钠辅助帕罗西汀对抑郁症患者血清BDNF、叶酸、Hcy及炎性因子影响 被引量:6

摘要 目的探讨左甲状腺素钠辅助帕罗西汀对抑郁症患者血清中脑源性神经营养因子(BDNF)、叶酸、同型半胱氨酸(Hcy)及血清炎性细胞因子[白介素-1β(IL-1β)、IL-6及肿瘤坏死因子-α(TNF-α)]的影响。方法选取2016年1月至2018年1月在湖北省武汉市第二精神病医院精神科接受治疗的抑郁症患者共160例,随机分为对照组(80例)和试验组(80例)。ELISA法测定治疗前后两组病人血清中BDNF、叶酸、Hcy及血清炎性细胞因子(IL-1β、IL-6及TNF-α)水平。汉密尔顿抑郁量表(hamilton depression scale,HAMD)和汉密尔顿焦虑量表(hamilton anxiety scale,HAMA)评估患者临床症状变化。结果经6周左甲状腺素钠辅助治疗后,试验组患者血清BDNF和叶酸水平较对照组显著增加(P <0. 05)。与对照组相比,左甲状腺素钠辅助治疗能够有效降低血清中Hcy及炎性细胞因子(IL-6及TNF-α)的水平(P <0. 05),但IL-1β没有明显改变(P> 0. 05)。左甲状腺素钠辅助治疗还能较对照组显著降低患者的HAMD和HAMA评分(P <0. 05)。结论左甲状腺素钠能够有效降低抑郁症患者体内Hcy及炎性细胞因子(IL-6及TNF-α)水平,提高患者血清BDNF和叶酸水平,改善患者的HAMD和HAMA评分,从而辅助参与抑郁症的治疗。 Objective To investigate the influences of levothyroxine sodium and paroxetine in treatment of patients with depressive disorder on serum brain-derived neurotrophic factor(BDNF),folic acid,homocysteine(Hcy)and serum inflammatory cytokines(IL-1β,IL-6 and TNF-α).Methods A total of 160 patients with depressive disorder who received routine treatment in Department of Psychiatry,Second Hospital from January 2016 to January 2018 in Wuhan City,Hubei Province were selected and divided into control group(80 cases)and intervention group(80 cases).Serum levels of BDNF,folic acid,Hcy and serum inflammatory cytokines were measured by the ELISA assay.Both the Hamilton depression scale(HAMD)and Hamilton anxiety scale(HAMA)were used to assess the change of clinical psychiatric symptoms.Results After 6 weeks of levothyroxine sodium treatment,the serum levels of BDNF and folic acid in intervention group were significantly higher than those in control group(P<0.05).The Hcy level and serum inflammatory cytokines(IL-6 and TNF-α)were significantly lower than in control group(P<0.05).There was no statistical significance in IL-1βbetween 2 groups(P>0.05).Levothyroxine sodium treatment also significantly reduced the HAMA and HAMD scores as compared to control group.Conclusion Levothyroxine sodium combined with paroxetine can effectively reduce the level of Hcy and serum IL-6 and TNF-αin patients with depressive disorder.levothyroxine sodium intervention also increase the serum BDNF and folic acid levels to improve the patient's HAMA and HAMD scores in the adjuvant treatment of depressive disorder.
