摘要
目的评价创面修复抗菌敷料联合卫生棉条在内科危重症中重度粪失禁相关性皮炎护理中的效果。方法选取我院内科危重症中重度粪失禁相关性皮炎患者120例,采用随机分组法分为A、B、C三组各40例。在相同皮肤护理方法基础上,A组肛周皮肤每2小时翻身时喷涂一次创面修复抗菌敷料,B组予卫生棉条塞肛每6小时更换一次,C组为A、B两组护理方法的组合,连续干预1周,观察三组肛周皮肤护理效果及皮肤护理频次。结果三组患者皮肤护理有效率比较差异有统计学意义(P <0. 05)且C组有效率优于A、B两组(P <0. 05)。三组患者1周内肛周皮肤护理次数比较,差异有统计学意义(F=135. 456,P=0. 000),再两两比较显示,B组及C组患者肛周皮肤护理次数明显少于A组,差异均有统计学意义(P <0. 05)。结论创面修复抗菌敷料联合卫生棉条在中重度粪失禁相关性皮炎护理中效果显著,能显著改善皮肤损伤症状,减少护理频次和工作量,提高护理工作效率,是一种有效的粪失禁相关性皮炎护理方法。
Objective To evaluate the nursing effects of wound-healing antibiotic dressings combined with tampons in critical patients with moderate-to-severe fecal incontinence associated dermatitis.Methods A total of 120 patients with moderate-to-severe fecal incontinence associated dermatitis treated in our hospital were randomly divided into A,B,and C groups,40 in each group.On the basis of the same skin care method,in the group A,patients'perianal skin was sprayed once every 2 hours with wound repair antibacterial dressing;patients in the group B were replaced with tampon in anal every 6 hours,and patients in the group C were treated with the integrated methods of the group A and B.After continuous intervention for 1 week,effects of perianal skin care and the frequency of skin care in the three groups were observed.Results The difference in skin care efficacy among the three groups was statistically significant(P<0.05)and the group C was better than group A and B(P<0.05).The frequency of perianal skin care was compared among the three groups of patients within 1 week.The difference was statistically significant(F=135.456,P=0.000).The comparison between every two groups showed that the frequency of perianal skin care in the group B and C was significantly less than that in the group A(P<0.05).Conclusions Wound-healing antibiotic dressings combined with tampons have a pronounced effect in the nursing of critical patients with moderate-to-severe fecal incontinence associated dermatitis.It could distinctly relieve the symptoms of skin lesions,reduce the nursing frequency,lessen the workload of nursing and improve the nursing efficiency.It shall be deemed as a highly effective method in the nursing of fecal incontinence associated dermatitis.
