摘要
目的总结经皮撬拨复位弹性髓内钉固定治疗儿童桡骨颈骨折的临床疗效。方法对24例桡骨颈JudetⅢ、Ⅳ型骨折患儿在C臂机间断透视下采用克氏针撬拨复位、弹性髓内钉固定,术后辅以石膏托外固定。术后4周去除石膏进行功能锻炼,术后3～6个月视骨折愈合情况取出弹性髓内钉。结果患儿均获得随访,时间8～24个月。骨折均获得骨性愈合。术后8个月根据Tibone-Stoltz评分标准评价肘关节功能疗效:优16例,良6例,可2例,优良率为22/24。患儿均未发生桡神经损伤、桡骨头缺血坏死、骨骺早闭、针道感染等并发症。结论经皮撬拨复位弹性髓内钉固定治疗儿童桡骨颈JudetⅢ、Ⅳ型骨折疗效满意,手术创伤小,并发症少,术后可较快恢复肘关节功能。
ObjectiveTo summarize the clinical effect of percutaneous poking reduction and elastic intramedullary nailing fixation in the treatment of pediatric radial neck fractures.MethodsTwenty-four children with Judet typeⅢandⅣradial neck fracture were treated with Kirschner wire poking reduction and elastic intramedullary nail fixation under C-arm intermittent fluoroscopy.Plaster support was used for external fixation after operation.The plaster was removed at 4 weeks after operation for functional exercises.Elastic intramedullary nails were removed in 3~6 months after operation depending on fracture healing.ResultsAll patients were followed up for 8~24 months.All fractures were healed.According to Tibone-Stoltz scoring criteria,elbow function was evaluated at 8 months after operation:excellent in 16 cases,good in 6,fair in 2.The excellent and good rate was 22/24.No complications such as radial nerve injury,ischemic necrosis of the radial head,premature closure of the epiphysis and infection of the pin tract occurred.ConclusionsKirschner wire percutaneous poking reduction and elastic intramedullary nail fixation for the treatment of Judet typeⅢandⅣradial neck fractures in children can provide satisfactory results,less surgical trauma,fewer complications,postoperative recovery of elbow joint function can be faster.
