摘要
目的探讨联合检测血清淀粉样蛋白A(SAA)和超敏C反应蛋白(hs-CRP)在小儿呼吸道感染性疾病早期鉴别诊断中的应用价值。方法根据病原体检测结果选取2016年8月~2018年3月我院儿科确诊收治的80例细菌感染的急性期呼吸道感染患儿作为细菌组,选取同期确诊收治的80例病毒感染的急性期呼吸道感染患儿作为病毒组,另选取80例健康儿童作为对照组。采用胶乳增强免疫比浊法和免疫荧光法分别测定比较各组的血清SAA和hs-CRP水平,计算SAA/hs-CRP值,分析联合检测血清SAA和hs-CRP的诊断价值。结果细菌组急性期的血清SAA、hs-CRP水平均明显高于病毒组和对照组,差异有统计学意义(P<0.05)。细菌组急性期的SAA/hsCRP水平高于对照组,但显著低于病毒组,差异有统计学意义(P<0.05)。病毒组急性期的血清SAA水平和SAA/hs-CRP明显高于对照组,差异有统计学意义(P<0.05),但两组的hs-CRP水平比较,差异无统计学意义(P>0.05)。SAA/hs-CRP诊断鉴别早期小儿呼吸道病毒感染的AUC面积最高,达到0.928,且诊断的灵敏度、特异度、阳性预测值以及阴性预测值均显著高于SAA、CRP检测(P<0.05)。结论联合检测血清SAA和hs-CRP水平更有助于小儿呼吸感染性疾病的早期鉴别诊断,具有较高的临床应用价值。
Objective To explore the application value of combined detection of serum amyloid A(SAA) and hypersensitive C-reactive protein(hs-CRP) in the early differential diagnosis of respiratory tract infections in children. Methods From August 2016 to March 2018, 80 children with acute respiratory infection of bacterial infection in our hospital were selected as the bacterial group. A total of 80 children with acute respiratory infection of viral infection at the same time in our hospital were selected as the viral group. Another 80 healthy children in our hospital were selected as the control group. The levels of serum SAA and hs-CRP were measured by latex enhanced immunoturbidimetry and immunofluorescence respectively. SAA/hs-CRP value was calculated and the diagnostic value of combined detection of serum SAA and hs-CRP was analyzed. Results The serum SAA and hs-CRP levels in the acute phase of the bacterial group were significantly higher than those of the virus group and the control group, with significant difference(P<0.05).The SAA/hs-CRP in the acute phase of the bacterial group was higher than that of the control group, but which was significantly lower than that of the control group, with significant difference(P<0.05). The serum SAA level and SAA/hs-CRP in the acute phase of the virus group were significantly higher than those of the control group, with significant difference(P<0.05), but there was no signifi cant difference in the levels of hs-CRP between the two groups(P>0.05). The AUC area of SAA/hs-CRP in the diagnosis and differential diagnosis of early respiratory tract virus infection in children was the highest, which was 0.928,and the sensitivity, specificity, positive predictive value and negative predictive value of SAA/hs-CRP were also significantly higher than those of SAA and CRP(P<0.05). Conclusion The combined detection of serum SAA and hs-CRP levels is helpful to the early differential diagnosis of respiratory infections in children and has a high clinical value.
