摘要
目的:探讨血液灌流(hemoperfusion,HP)治疗氧化乐果中毒患者的临床效果。方法:70例氧化乐果中毒患者,均为口服中毒入院接受急救。随机数表法将其分为HP治疗组和传统治疗组(非HP组),每组各35例。观察两组患者昏迷时间、阿托品用量、住院时间、血液胆碱酯酶(cholinesterase,Ch E)活性变化、血液毒物清除情况等,比较两组预后情况。结果:HP组昏迷时间、阿托品用量、住院时间分别较非HP组低(P <0. 05),HP组治疗1 d、3 d时血清Ch E活性较非HP组升高更为显著(P <0. 05),HP组治疗3 d时血液氧化乐果浓度明显低于非HP组(P <0. 05),两组存活率比较,差异无统计学意义(P>0. 05),但HP组反跳、IMS发生率显著低于非HP组(P <0. 05)。结论:HP对恢复氧化乐果中毒患者血液Ch E活性和清除血液毒物浓度效果明显,对促进病情转归、优化临床治疗等应用价值显著。
Objective:To investigate the clinical effect of Hemoperfusion(HP)on Omethoate intoxication.Methods:70 cases of Omethoate intoxication were selected for oral first aid.The random number table was divided into HP treatment group and traditional treatment group(non HP group).There were 35 cases in each group.The patients’coma time,atropine dosage,hospitalization time,blood cholinesterase(ChE)activity and blood poison clearance were observed in two groups,and the prognosis of the two groups was compared.Results:The time of coma,the dosage of atropine and the time of hospitalization in the group HP were lower than those in the non HP group(P<0.05).The serum ChE activity in the group HP was significantly higher than that in the non HP group at 1 and 3 d after treatment(P<0.05),the blood Omethoate concentration in HP group was significantly lower than that in non-HP group at 3 d after treatment(P<0.05).There was no significant difference in survival rate between the two groups(P>0.05).The incidence of rebound and IMS in group HP was significantly lower than that in non HP group(P<0.05).Conclusion:HP has obvious effect on the recovery of ChE activity and the concentration of blood poison in patients with oodiomethoate poisoning.It is of great value in promoting the prognosis of the disease and optimizing the clinical treatment.
