摘要
随着中风病的增加,中风后抑郁的发病率也逐年上升。中风后抑郁主要为情志抑郁,悲观失望,对生活失去信心的一类精神情志类疾病。中风后抑郁的发病率高,病死率高,致残率高,严重影响人类的工作,学习与生活。中医学认为中风后抑郁的病理基础为正气亏虚,继而发生气滞、血瘀、痰凝的病理变化。而现代医学对中风后抑郁的病因尚不明确,其主要病机主要为内源性机制学说和反应性机制学说。中医学主要将抑郁分为肝气郁结、痰气郁结、血行瘀滞、心脾两虚和脾肾阳虚这五个证型。并通过辨证用药治疗,根据患者具体情况进行剂型、剂量的调整。西医主要通过抗抑郁药物来控制与稳定病情,常见的抗抑郁药分为:三环类、单胺氧化酶抑制剂、新型抗抑郁药与其他类型的抗抑郁药。随之而来的各类副作用同样也成为困扰人类的一大难题。随着医学的快速发展,人类的诉求也逐渐提高,渴望有一种绿色、健康的疗法作为现代医学的替代。而针灸逐渐在医学中兴起,其治疗的有效性,无副作用被患者广为接纳。针灸治疗疾病主要通过神经、免疫、内分泌机制三个方面起作用,而针灸治疗中风后抑郁主要为调节HPA轴与神经内分泌的功能。本文查阅了有关针灸治疗中风后抑郁的临床文献报道,结果显示其疗效显著。且与西药相比,具有安全性高,副作用少,经济方便的优点,值得临床推广使用。
With the increasing of stroke,the incidence of depression after stroke has increased year by year.Post stroke depression is mainly emotional depression,pessimism,and loss of confidence in life.The incidence of post-stroke depression is high,the mortality rate is high,and the rate of disability is high,which seriously affects human's work,study and life.According to Chinese medicine,the etiology of depression after middle and post syndrome is based on the pathological changes of qi deficiency,blood stasis and phlegm coagulation.The etiology of post-stroke depression is not clear in modern medicine.Its main pathogenesis is endogenous mechanism and reactive mechanism.Traditional Chinese medicine mainly divides depression into two syndromes:stagnation of liver qi,stagnation of phlegm Qi,stagnation of blood circulation,deficiency of heart and spleen,and deficiency of spleen and kidney yang.And through the dialectical diagnosis,medication,dosage form and dosage adjustment are according to the specific circumstances of patients.Western medicine treatment is mainly controlled and stabilized by antidepressant drugs.The common antidepressants are divided into:Tricyclic,monoamine oxidase inhibitors,new antidepressants and other types of antidepressants.All kinds of side effects have also become a big problem for human beings.With the rapid development of medicine,the demands of human beings are gradually improving.They are eager to have a green and healthy therapy as a substitute for modern medicine.Acupuncture and moxibustion are gradually emerging in medicine.The effectiveness of treatment and no side effects are widely accepted by patients.The mechanism of acupuncture treatment of disease is mainly divided into three aspects:nerve,immunity and endocrine mechanism,and the mechanism of acupuncture treatment for post stroke depression is to regulate the HPA axis and regulate the function of neuroendocrine.This article reviews the clinical literature on acupuncture and moxibustion for post-stroke depression.Compared with western medicine,it has the advantages of high safety,less side effects and economic convenience,which is worthy of clinical promotion.
