摘要
目的研究以他汀类为基础的降脂药物联合应用发生不良反应的相关危险因素。方法选取2017年1月1日～12月31日在本院以他汀类为基础的降脂药物联合应用治疗的400例患者作为研究对象,对患者的临床资料进行回顾性分析,观察患者发生的药物不良反应种类及相关因素。结果 400例患者中,共出现65例不良反应,其中以骨骼肌系统损伤、肝功能损伤为主。单因素分析结果提示,女性患者的不良反应发生率高于男性,差异有统计学意义(P<0.05);年龄>70岁患者的不良反应发生率高于年龄≤70岁,差异有统计学意义(P<0.05);合并3种基础疾病患者的不良反应发生率高于合并1、2种基础疾病患者,差异有统计学意义(P<0.05);合并使用药物4种及以上患者的不良反应发生率高于合并1、2、3种药物患者,差异有统计学意义(P<0.05)。多元回归性分析显示,女性、年龄>70岁、合并3种基础疾病及合并药物4种及以上为他汀类药物发生不良反应的危险因素(P<0.05)。结论以他汀类为基础的降脂药物联合应用,会导致发生不良反应,需对女性、高龄、联合多种药物及基础疾病较多患者给予高度重视。
Objective To study the risk factors associated with adverse reactions in combined usage of Statin-based lipid-lowering drugs.Methods Four hundred patients who were treated with Statin-based lipid-lowering drugs in combination therapy in our hospital from January 1 to December 31,2017 were selected as subjects.The clinical data of the patients were retrospectively analyzed in order to observe the types of adverse drug reactions and related factors.Results A total of 65 adverse reactions occurred in 400 patients,mainly including skeletal muscle system and liver function damage.The results of single factor analysis suggest that the incidence of adverse reactions in women was higher than that in men,the difference was statistically significant(P<0.05),the incidence of adverse reactions in patients aged > 70 years old was higher than that of patients aged less than or equal to 70 years old,and the difference was statistically significant(P<0.05),the incidence of adverse reactions in patients with 3 basic diseases was higher than that in patients with 1 or 2 basic diseases,the differences were statistically significant(P<0.05),the incidence of adverse reactions in patients with 4 or more drugs combined was higher than that in patients with 1,2 or 3 drugs combined,the differences were statistically significant(P<0.05).Multivariate regression analysis showed that women,aged >70 years old,combined with 3 basic diseases and 4 or more drugs were risk factors for adverse reactions to Statins(P<0.05).Conclusion The combination of Statin-based lipid-lowering drugs can lead to adverse reactions,and it is necessary to pay great attention to female patients at advanced age with multiple underlying diseases and combined usage of drugs.
