摘要
目的分析miR-182和miR-331-3p在膀胱癌患者血清中的表达情况,并评估二者对膀胱癌诊断及预后判断的价值。方法选择膀胱癌患者183例作为膀胱癌组,另选同期健康者120例作为对照组。采用实时定量PCR分析血清中miR-182和miR-331-3p相对表达;受试者工作特征(ROC)评估miR-182和miR-331-3p对膀胱癌的诊断价值; Kaplan-Meier法构建生存曲线。结果与对照组比较,膀胱癌患者血清miR-182和miR-331-3p相对表达水平均显著升高(P <0. 05)。miR-182与肿瘤分期、淋巴结转移均有关(P <0. 05),而miR-331-3p与肿瘤大小、肿瘤分期、淋巴结转移均有关(P <0. 05)。miR-182对膀胱癌患者诊断的AUC为0. 912,敏感性为78. 65%,特异性为91. 60%。miR-331-3p对膀胱癌患者诊断的AUC为0. 891,敏感性为79. 62%,特异性为89. 50%。miR-182高表达组总体生存率显著低于miR-182低表达组(χ2=5. 808,P=0. 016)。miR-331-3p高表达组总体生存率显著低于miR-331-3p低表达组(χ2=4. 712,P=0. 030)。miR-182和miR-331-3p水平是影响膀胱癌患者预后的独立危险因素(P <0. 05)。结论 miR-182和miR-331-3p在膀胱癌患者血清中的表达上调,与临床病理特征明显相关,可能是膀胱癌的潜在诊断标志物,且是膀胱癌患者预后的独立预测因子。
Objective To analyze the expression of miR-182 and miR-331-3p in the serum of patients with bladder cancer,and to evaluate their diagnostic value and prognostic value in bladder cancer.Methods A total of 183 patients bladder cancer were enrolled as case group,the other 120 healthy subjects were enrolled as control group.Real-time quantitative PCR was used to analyze the relative expression of miR-182 and miR-331-3p in serum.The receiver operating characteristic(ROC)was used to evaluate the diagnostic value of miR-182 and miR-331-3p in bladder cancer.The survival curve was established by means of Kaplan-Meier method.Results As compared with those in control group,the relative expression levels of miR-182 and miR-331-3p in case group were significantly increased(P<0.05).The miR-182 was closely correlated with tumor stage and lymph node metastasis(P<0.05),while miR-331-3p was associated with tumor size,tumor stage and lymph node metastasis(P<0.05).The diagnostic AUC of miR-182 in diagnosis of bladder cancer was 0.912,with sensitivity and specificity being 78.65%and 91.60%,respectively.The AUC of miR-331-3p in diagnosis of bladder cancer was 0.891,with sensitivity and specificity being 79.62%and 89.50%,respectively.The overall survival rate in miR-182 high-expression group was significantly lower than that in miR-182 low-expression group(χ^2=5.808,P<0.05).The overall survival rate in miR-331-3p high expression group was significantly lower than that in miR-331-3p low expression group(χ^2=4.712,P<0.05).The levels of miR-182 and miR-331-3p were independent risk factors influencing the prognosis of patients with bladder cancer(P<0.05).Conclusion The upregulation of miR-182 and miR-331-3p in the serum of patients with bladder cancer is closely related to clinicopathological features,which may be a potential diagnostic marker of bladder cancer and is an independent predictor of prognosis of patients with bladder cancer.
