摘要
目的探讨针对性个性化综合护理对胎膜早破产妇分娩的新生儿发生感染的影响。方法选取住院分娩的胎膜早破产妇共400例,患者采用随机数字表法分为观察组和对照组,每组200例。对照组给予临床常规护理,观察组在对照组基础上给予针对性个性化综合护理,干预前后发放焦虑自评量表(SAS)和抑郁自评量表(SDS)评估产妇焦虑、抑郁程度,比较产妇并发症发生率及新生儿结局。结果观察组治疗后SAS评分和SDS评分低于对照组,差异有统计学意义(P <0. 05);观察组阴道分娩率为79. 0%高于对照组的60. 0%,差异有统计学意义(P <0. 05);观察组早产、产褥感染、产妇感染发病率分别为10. 0%、1. 5%、2. 0%低于对照组的18. 5%、5. 0%、6. 5%,差异有统计学意义(P <0. 05);观察组围生儿感染、新生儿肺炎发生率分别为6. 5%、3. 0%低于对照组的12. 5%、7. 5%,差异有统计学意义(P <0. 05)。结论针对性个性化综合护理可改善胎膜早破孕妇焦虑、抑郁情绪,提高剖宫产率,降低孕妇并发症及新生儿感染发生率。
Objective To investigate the effects of targeted and individualized comprehensive nursing on infection in neonates born by parturients with premature rupture of membranes. MethodsA total of 400 parturients with premature rupture of membranes who were hospitalized and delivered in our hospital were enrolled in the stusy. They were divided into observation group ( n =200) and control group ( n =200) by random number table method.The 200 parturients in control group were treated by routine nursing,while the other 200 parturients in observation group,on the basis of control group,were treated by targeted and individualized comprehensive nursing.The degree of maternal anxiety and depression was evaluated by Self-rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS).The incidence rate of maternal complications and neonatal outcomes were observed and compared between the two grousp. ResultsAfter treatment the SAS scores and SDS scores in observation group were significantly lower than those in control group ( P < 0.05).The vaginal delivery rate in observation group was significantly higher than that in control group (79.0% vs 60.0 %, P <0.05).The incidence rates of preterm delivery,puerperal infection and maternal infection in observation group (10.0%,1.5% and 2.0%) were significantly lower than those in control group (18.5%,5.0% and 6.5%, P <0.05).The incidence rates of perinatal infection and neonatal pneumonia in observation group (6.5% and 3.0%) were significantly lower than those in control group (12.5% and 7.5%, P <0.05). ConclusionThe targeted and individualized comprehensive nursing can relieve the anxiety and depression of pregnant women with premature rupture of membranes,improve the cesarean section rate,and reduce the incidence of maternal complications and neonatal infection.
