类风湿关节炎患者血清淀粉样蛋白A和C反应蛋白水平与疾病活动度的关系 被引量:12

Relationship between Serum Amyloid A and C-reactive Proteinin Patients with Rheumatoid Arthritis and Activity of Disease

摘要 目的探讨类风湿关节炎(RA)患者血清淀粉样蛋白A(SAA)和C反应蛋白(CRP)与疾病活动度的相关性。方法检测210例RA患者(RA组)、100健康体检者(对照组)的SAA,CRP及红细胞沉降率(ESR)等实验室指标,根据疾病活动指数(DAS 28)将RA组分为活动期(97例,DAS 28≥2.6分)和缓解期(113例,DAS 28<2.6分),根据影像学结果分为骨损伤88例和无骨损伤122例。比较不同病情活动度患者SAA和CRP水平变化,及SAA,CRP与DAS 28的相关性。结果缓解期RA组血清SAA水平(136.07±22.14μg/L)显著高于对照组(108.54±13.28μg/L),差异有统计学意义(t=10.75,P<0.05),而两组CRP水平(4.63±2.16mg/L vs 4.16±1.28mg/L)比较,差异无统计学意义(t=0.78,P>0.05)。活动期RA组血清SAA(611.46±62.77μg/L),CRP(34.63±4.88mg/L)显著高于对照组和缓解期RA组,差异有统计学意义(t=69.65,55.13,P<0.05)。骨损伤患者血清SAA(587.23±60.36μg/L vs 147.55±13.18μg/L),CRP(30.48±4.27mg/L vs 4.85±1.38mg/L)较无骨损伤患者明显升高,差异有统计学意义(t=67.15,50.27,P<0.05)。缓解期患者DAS 28评分与SAA呈显著正相关(r=0.372,P<0.05),而与CRP无显著相关性(r=0.289,P>0.05)。活动期患者DAS28评分与SAA,CRP均呈显著正相关(r=0.414,0.342,均P<0.05)。RA患者血清SAA与CRP呈显著正相关(r=0.428,P<0.05)。结论 SAA在RA患者中明显升高,与CRP,DAS28评分相关性好,SAA联合CRP可作为疾病活动状态的敏感指标。 Objective To explore the correlation of serum amyloid A(SAA)and C-reactive protein(CRP)and activity of rheumatoid arthritis(RA).Methods Laboratory indexes such as SAA,CRP and blood sedimentation(ESR)were detected in210 patients with RA(RA group),100 healthy physical examination(control group).According to the disease activity index(DAS 28),RA group was divided into active period(n=97,DAS 28≥2.6)and remission period(n=113,DAS 28<2.6).According to the imaging results,RA group was divided into patients with bone erosion(n=88)and patients without bone erosion(n=122).The changes of SAA and CRP levels in patients with different disease activity and the correlation between SAA and CRP and DAS 28 were compared.Results The level of SAA(136.07±22.14μg/L)in RA group was significantly higher than that in control group(108.54±13.28μg/L)(t=10.75,P<0.05)respectively,while CRP level(4.63±2.16 mg/L vs 4.16±1.28 mg/L)was not statistically significant between two groups(t=0.78,P >0.05)respectively.The levels of SAA(611.46±62.77μg/L)and CRP(34.63±4.88 mg/L)in active RA group were significantly higher than those in control group and remission RA group(t=69.65,55.13,P<0.05)respectively.The levels of SAA and CRP were significantly higher in patients with bone injury(587.23±60.36μg/L,30.48±4.27 mg/L)than those with out bone injury(147.55±13.18μg/L),(4.85±1.38 mg/L)(P<0.05)respectlvey.The DAS28 score in remission patients showed a significant positive correlation with SAA(r=0.372,P<0.05),but no significant correlation with CRP(r=0.289,P >0.05).The DAS28 score of active patients was significantly positively correlated with serum SAA and CRP(r=0.414,0.342,P<0.05).SAA and CRP of RA patients showed significant positive correlation(r=0.428,P<0.05).Conclusion SAA is significantly increased in RA patients,and correlated well with CRP and DAS 28 scores.SAA combined with CRP could be used as a sensitive indicator of disease activity status.
