摘要
目的探讨辅助生殖技术过程中简单的精子洗涤程序对高龄男性精子功能的影响。方法选取2018年3月1日至6月1日在宁夏医科大学总医院生殖中心门诊就诊的男性不育患者68例,将其分为2组:低年龄组(20~<40岁,n=32)和高年龄组(40~50岁,n=36)。每份精液样本依据世界卫生组织标准检测精液的各项常规参数和精子功能,处理过程按照《世界卫生组织人类精液检查与处理实验室手册(第5版)》推荐的简单洗涤程序进行操作。分别检测精液处理前2组精液体积、精子浓度、精子活力、精子正常形态率,以及处理前后精子功能指标包括[精子DNA断裂指数(DFI)、精子线粒体膜电位(MMP)及顶体反应(AR)],并进行结果的比较、分析。结果精子常规参数结果显示,高年龄组精子正常形态率低于低年龄组,差异有统计学意义(P<0.05);但2组在精液体积、精子浓度、精子活力方面比较,差异均无统计学意义(P>0.05)。2组经过简单洗涤处理后,精子DFI值均明显升高,且高年龄组升高幅度高于低年龄组;MMP值显著降低,且高年龄组的降低幅度高于低年龄组,差异均有统计学意义(P<0.05)。2组AR值经处理后显著降低,但2组间的降低幅度比较,差异无统计学意义(P>0.05)。结论高龄对精液体积、精子浓度、精子活力无明显影响,但对精子正常形态率影响较大;在高龄组精子处理过程中精子的DFI、MMP变化较大,需要进一步优化处理洗涤过程,以减少对精子的负面影响。
Objective To explore the effect of simple washing procedure on sperm function in elderly men during assisted reproductive technology.Methods A total of 68 male infertility patients admitted to the outpatient department of the reproductive center of Ningxia Medical University General Hospital from March 1 to June 1,2018 were divided into two groups:the low age group(20-<40 years old,n=32)and the high age group(40-50 years old,n=36).Each semen sample was used to test the parameters of the semen routine and the sperm function,and the process was operated according to the simple washing procedure recommended by WHO Laboratory Manual for the Examination and Processing of Human Semen(5th edition).The semen volume,sperm concentration,sperm motility,sperm normal form rate of semen samples in each group before semen treatment were detected,and before and after semen were measured,and sperm function[including sperm DNA fracture index(DFI),sperm mitochondrial membrane potential(MMP),and acrosome reaction(AR)]before and after semen treatment were detected,and the results were compared and analyzed.Results The results of sperm routine parameters showed that the normal sperm morphology of the high age group was lower than that of the low age group,the difference was statistically significant(P<0.05),but there were no significant difference between the two groups in semen volume,sperm concentration and sperm motility(P >0.05).After the two groups were treated with simple washing,the DFI value of sperm increased significantly,and the increase in the high age group was higher than that in the low age group;the MMP values of sperm in the two groups were significantly reduced after treatment,and the decrease in the high age group was higher than that in the low age group,the differences were statistically significant(P<0.05).The AR values in the two groups were significantly reduced after treatment,but there was no significant difference between the two groups(P >0.05).Conclusion The age has no obvious influence on the semen volume,sperm concentration and sperm motility,but has great influence on the sperm normal morphology rate.In the process of sperm treatment in the high age group,the DFI and MMP of sperm change greatly,so the process of treatment should be further optimized to reduce the negative effects on sperm function.
