血清25羟维生素D_3与甲状腺自身抗体及甲状腺功能的关系 被引量:14

摘要 目的研究血清25羟维生素D_3[25(OH)D_3]与甲状腺自身抗体及甲状腺功能的相关性。方法根据研究标准纳入501例,收集纳入者一般资料、性别、年龄、血清25(OH)D_3、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(A-TPO)、甲状腺球蛋白抗体(A-TG),比较不同25(OH)D_3营养状态下的甲状腺自身抗体阳性率的差异;采用二元Logistic回归分析25(OH)D_3缺乏与甲状腺自身抗体阳性的关系;根据甲状腺功能状态将确诊为桥本氏甲状腺炎(HT)的人群分为不同进展期,比较不同进展期25(OH)D_3水平、不同25(OH)D_3营养状态与不同进展期的HT患者的甲状腺功能状态的关系。结果不同25(OH)D_3营养状态组间的A-TPO水平差别无统计学意义(P>0.05),A-TG水平差别有统计学意义(P<0.05);A-TG的阳性率、A-TPO和A-TG同时阳性率的差别有统计学意义(P<0.05)。二元Logistic回归分析显示,血清25(OH)D_3的增加可减少A-TPO阳性、A-TG阳性、A-TPO和(或)A-TG阳性的可能性(OR值为0.972,0.964,0.962);与25(OH)D_3≥50 nmol/L比较,25(OH)D_3<50 nmol/L可增加A-TPO和(或)A-TG阳性的风险(OR值为2.514);不同25(OH)D_3营养状态与不同进展期的HT患者的甲状腺功能状态的Gamma关联分析显示,随着25(OH)D_3营养状态减低,HT患者的甲状腺功能状态逐渐趋向于甲状腺功能减退。结论低水平的25(OH)D_3与甲状腺自身抗体阳性相关,与HT患者的甲状腺功能逐渐减退相关。 Objective To study the relationship between 25(OH)D 3 and thyroid autoantibodies and thyroid function.Methods According to the study criteria,501 patients were included,and the general data on the participants were collected.The participants were studied for 25(OH)D 3 and thyroid function.Comparison of positive rates of thyroid autoantibodies in different 25(OH)D 3 nutritional status were performed.Study was conducted on the effect of 25(OH)D 3 deficiency on positive thyroid autoantibodies by binary logistic regression analysis.According to the thyroid function status,the patients diagnosed with Hashimoto's thyroiditis(HT)were divided into different stages,and the relationship between the level of 25(OH)D 3 and the nutritional status of 25(OH)D 3 and thyroid function status of HT patients were analyzed.Results This study found no significant difference in the level of A-TPO between different 25(OH)D 3 nutritional status groups.The difference in the level of A-TG was statistically significant.The positive rate of A-TG between different 25(OH)D 3 nutritional status groups,and the positive rate of A-TPO and A-TG were different.The differences were statistically significant.Binary Logistic regression analysis showed that an increase in serum 25(OH)D 3 reduced the likelihood of A-TPO positive,A-TG positive,A-TPO and or A-TG positive,and the OR values were(0.972,0.964,0.962).Compared with the level of serum 25(OH)D 3 greater than 50 nmol/L,when the level of serum 25(OH)D 3 was less than 50 nmol/L,the risk of A-TPO and or A-TG positive could be increased,and the OR value was 2.514.Gamma correlation analysis showed that as the nutritional status of 25(OH)D 3 was reduced,the thyroid function status of HT patients tended to be hypothyroidism.Conclusions The low level of 25(OH)D 3 is positively associated with thyroid autoantibodies.The low level of 25(OH)D 3 is also associated with decreased thyroid function in patients with HT.
