头颈动脉CT血管成像脑动脉强化的相关体质因素 被引量:12

摘要 目的探讨影响患者头颈联合CT血管成像(CTA)脑动脉强化阈值一致性的相关因素,并确定最佳延迟扫描时间,优化对比剂注射及扫描方案。资料与方法回顾性收集临床拟诊为缺血性脑血管病行头颅CTA检查的108例患者,均行头颅CT灌注及头颅增强检查,并采用统一的碘注射流速及扫描方案。分析身高、体重、体表面积、体重指数以及心率与输入动脉强化阈值的相关性,计算输入动脉(颈内动脉)及输出静脉(上矢状窦)的平均达峰时间差。结果患者各种因素与动脉强化程度的相关性为:体表面积(r=-0.565,P<0.001)>体重(r=-0.556,P<0.001)>体重指数(r=-0.448,P<0.001)>身高(r=-0.429,P<0.001),与心率无相关性(r=0.093,P>0.05)。输入动脉及输出静脉的平均达峰时间差为6.42 s。结论为提高不同患者头颈联合CTA脑动脉强化阈值的一致性,可按体表面积或体重估算对比剂的用量。注射对比剂后CTA扫描时间不应超过6.42 s。 Purpose To explore the related factors affecting patients' head and neck combined CT angiography (CTA) cerebral arterial enhanced threshold consistence and confirm optimal delayed scanning time to optimize contrast agent injection protoc ol and scanning protocol. Materials and Methods One hundred and eight patients who were intended to be diagnosed ischemic cerebrovascular diseases and received head CTA examination were retrospectively collected. 256 rows spiral CT was taken for head CT perfusion and head enhancement examination and unified iodine injection flow rate and scanning protocol were adopted. Linear regression equation was applied to analyze height, weight, body surface area, body mass index and correlation of heart rate and input arterial enhancement threshold. Mean peak time difference of input arteries (internal carotid arteries) and output veins (superior sagittal sinus) was calculated. Results Correlation sequence of each factor with arterial enhancement degree of the patients came as follows: body surface area ( r =-0.565, P <0.001) was greater than weight ( r =-0.556, P <0.001) which was greater than body mass index ( r =-0.448, P <0.001) which was greater than height ( r =-0.429, P <0.001). There was no correlation with heart rate ( r =0.093, P >0.05). Mean peak time difference of input arteries and output veins was 6.42 s. Conclusion To improve consistency of head and neck combined CTA brain cerebral arterial enhancement threshold of different patients, dosage of cont rast agent could be estimated with body surface area or body weight. CTA scanning time after contrast agent injection should not exceed 6.42 s.
