摘要
目的探讨非老年人肺部真菌感染的临床特点、真菌感染类型及预后情况。方法回顾性分析我院2012年8月—2017年8月住院的32例非老年人肺部真菌感染患者的基本资料、基础疾病、临床表现、病原学检查、影像特点及治疗与转归。结果 32例患者中,肺恶性肿瘤6例,肺结核4例,慢性阻塞性肺疾病6例,血液系统肿瘤2例,自身免疫性疾病3例,艾滋病2例,糖尿病4例,无基础疾病5例。真菌感染类型以曲霉菌、隐球菌及念珠菌为主,极少数为马尔尼菲青霉菌。胸部CT表现为曲霉球4例,空气新月征2例,晕轮征4例,实变影5例,单发性肺结节8例,多发性肺结节2例,支气管肺炎3例,中央型支气管扩张4例。32例对抗真菌治疗反应情况,其中好转及痊愈27例,无变化4例,恶化1例。结论非老年人肺真菌感染大部分合并一些基础疾病,主要以曲霉菌和隐球菌感染为主,胸部影像学无明显特异性表现,对抗真菌治疗效果较好。
Objective To investigate the clinical features,the types of fungi and the prognosis of pulmonary fungal infection in non-elderly.Methods The retrospective analysis was conducted in collecting the clinical information,including basic document,basic diseases,clinical manifestations,aetiology,imaging and treatment.A total of 32 nonelderly patients were collected from the First Affiliated Hospital of Xiamen University from August 2012 to August 2017 who were diagnosed as pulmonary fungal infection.Results Among 32 patients,there were 6 cases with lung cancer,4 cases with pulmonary tuberculosis,6 cases with chronic obstructive pulmonary disease,2 cases with hematological malignancy,3 cases with autoimmune disease,2 cases with HIV,4 cases with diabetes and 5 cases without basic diseases.The main types of fungal infection were Aspergillus,Cryptococcus and Candida,very few was Penicillium marneffei.There were various manifestations in chest CT image,including 4 cases of aspergilloma,2 cases of air crescent sign,4 cases of halo sign,5 cases of consolidation,8 cases of single nodule,2 cases of multiple nodules,3 cases of bronchopneumonitis and 4 cases of central bronchiectasis.Most patients were responsed to antifungal drugs well,27 cases were cured or improved,there were no response in 4 cases,and 1 case deteriorated.Conclusion Most pulmonary fungal infection patients are complicated with some basic diseases.The main fungi are Cryptococcus and Aspergillus.There are no specific signs in imaging and the therapeutic response is very well in those non-elderly.
