摘要
目的分析帕金森病(PD)患者血尿酸水平与认知功能、炎性因子及神经细胞因子的相关性。方法选择2015年2月-2017年12月重庆市人民医院神经内科首次诊断为PD的患者作为PD组(n=100),同期在医院体检的健康志愿者作为健康对照组(n=80)。采集2组空腹血测定尿酸、炎性因子、神经细胞因子的水平,采用蒙特利尔认知功能评估(MoCA)量表评价认知功能。结果 PD组患者的血尿酸、BDNF、GDNF、EGF、IGF-1的水平以及认知功能评分均低于健康对照组(t/P=15. 839/0. 000、23. 020/0. 000、15. 348/0. 000、20. 210/0. 000、11. 135/0. 000、19.313/0.000),血IFN-γ、IL-1β、IL-6、YKL-40、sTNFR-1的水平高于健康对照组(t/P=26. 466/0. 000、40. 910/0. 000、27.475/0.000、19.761/0.000、14.311/0.000);PD组患者的血尿酸水平与IFN-γ、IL-1β、IL-6、YKL-40、sTNFR-1水平呈负相关(r/P=-0.614/0.007、-0.551/0.014、-0.476/0.022、-0.535/0.016、-0.418/0. 028),与BDNF、GDNF、EGF、IGF-1的水平以及认知功能评分呈正相关(r/P=0. 552/0. 014、0. 489/0. 020、0.452/0. 025、0. 591/0. 011、0. 394/0.031)。结论帕金森病患者血尿酸水平降低与认知功能损害、炎性因子及神经细胞因子异常分泌有关。
Objective To analyze the correlation between serum uric acid level and cognitive function,inflammatory factors and neurocytokines in patients with Parkinson's disease (PD). Methods From February 2015 to December 2017,patients with first diagnosed PD in the Department of Neurology of Chongqing People's Hospital were selected as PD group ( n =100),and healthy volunteers who underwent physical examination in the same period were selected as healthy control group ( n =80). The levels of uric acid,inflammatory factors and neurocytokines were measured in two groups of fasting blood. The cognitive function was evaluated by Montreal Cognitive Function Assessment (MoCA). Results The levels of serum uric acid,BDNF,GDNF,EGF,IGF-1 and cognitive function scores in the PD group were lower than those in the healthy control group ( t/P =15.839/0.000, t/P =23.020/0.000, t/P =15.348/ 0.000, t/P =20.210/0.000, t/P =11.135/0.000, t/P = 19.313/0.000),the levels of blood IFN-γ,IL-1β,IL-6,YKL-40,sTNFR-1 were higher than healthy controls ( t/P =26.466 /0.000, t/P =40.910/0.000, t/P =27.475/0.000, t/P =19.761/0.000, t/P =14.311/0.000);blood uric acid level and IFN-γ in PD patients,There was a negative correlation between IL-1β,IL-6,YKL-40 and sTNFR-1 levels ( r/P =-0.614 /0.007, r/P =-0.551/0.014, r/P =-0.476/0.022, r/P =-0.535/ 0.016, r/P =-0.418/0.028),positively correlated with the levels of BDNF,GDNF,EGF,IGF-1 and cognitive function scores ( r/P =0.552/0.014, r/P =0.489/0.020 , r/P =0.452/0.025, r/P =0.591/0.011,r/P =0.394/0.031). Conclusion The decrease of serum uric acid level in patients with Parkinson's disease is related to cognitive impairment,abnormal secretion of inflammatory factors and neurocytokines.
