摘要
目的探讨桥接组合式内固定系统治疗复杂肱骨近端骨折的临床效果。方法回顾性分析本院骨科自2013年1月至2017年1月收治的26例使用桥接组合式内固定系统治疗的复杂肱骨近端骨折患者的临床资料,其中男15例,女11例;年龄32~68岁,平均年龄(54.6±9.5)岁。其中Neer分型Ⅳ型8例,Neer分型Ⅲ型18例,评估手术时间、术中失血量、患肢肩关节功能评分、手术并发症发生情况。结果 26例患者均获得随访,随访时间10~26个月,平均(16.6±4.5)个月。26例患者手术时间85~171 min,平均(119.5±23.2) min;术中失血量为180~600 mL,平均(365.0±97.7)mL。在随访过程中,3个月内25例均获得骨性愈合,1例患者术后出现肱骨头缺血坏死。肩关节功能Neer评分为63~94分,平均(85.2±6.8)分,优良率为84.6%。结论桥接组合式内固定系统应用于复杂肱骨近端骨折治疗,可根据不同的骨折形态选择合适的多维固定,具有固定方式灵活、内固定效果可靠、并发症少等优点,是一种治疗复杂肱骨近端骨折的有效、安全的治疗方式。
Objective To explore the clinical effect of bridge combined internal fixation system for the treatment of complex proximal humerus fractures.Methods A retrospective study was conducted of 26 patients with complex proximal humerus fractures who treated with bridge combined internal fixation system from January 2013 to January 2017.They were 15 men and 11 women,aged from 32 to 68 years,with an average age of(54.6±9.5)years.Fractures were classified according to Neer’s type,including 8 cases of typeⅣ,18 cases of typeⅢ.To evaluate the operation time,intraoperative blood loss,shoulder joint function score and complications.Results All 26 patients were followed up for 10~26 months,with an average of(16.6±4.5)months.The operation time is 85~171 min,with an average of(119.5±23.2)min;the intraoperative blood loss is 180~600 mL,with an average of(365.0±97.7)mL.In the follow-up process,25 cases obtained osseous healing within 3 months,1 case occurred the humerus head ischemic necrosis.The average Neer’s shoulder function score of 26 patients was(85.2±6.8)points(63~94 points),the excellent and good rate was 84.6%.Conclusion Bridge combined internal fixation system applied in the treatment of proximal humerus complex fractures,with advantage of multi-dimensional fixation,flexible fixation method,reliable internal fixation effect and less complications,is a kind of effective and safe treatment of proximal humerus.
