摘要 目的探讨Graf法超声在新生儿髋关节发育不良研究中的初步应用。方法选取2015年6月至2018年6月我院新生儿儿科病房收治的疑似髋关节发育不良(DDH)的新生儿680例作为研究对象,分别采用Graf法超声和X线拍片诊断。对超声诊断和X线诊断资料进行统计学对比分析,评价两种方法在新生儿髋关节发育不良中的应用。结果采用Graf法超声检测出DDH阳性65例(9. 56%),其中43例髋关节发育不良,14例髋关节半脱位,8例全脱位; X线检测DDH阳性45例(6. 62%),其中26例髋关节发育不良,14例髋关节半脱位,5例髋关节全脱位。阳性率对比具有统计学差异(P<0. 05)。X线判断DDH发育不良少17例,DDH全脱位少3例,1个月后通过Graf法超声和X线复查显示,17例确为DDH发育不良,3例确为DDH全脱位。结论新生儿早期髋关节异常检查中,应用Graf法超声检查,能够更好地观察髋关节剖面结构,相比于X线检查,能够降低漏诊,为DDH提供有效依据和参考。 Objective To explore the preliminary application of Graf ultrasound in hip dysplasia of newborns.Methods 680 cases of newborns with suspected developmental dysplasia of the hip(DDH)treated in neonatal pediatric ward of our hospital from June 2015 to June 2018 were selected and given the Graf ultrasound and X-ray film diagnosis.The data of ultrasonic and X-ray diagnosis were statistically compared and analyzed to evaluate the application of two methods in hip dysplasia of newborns.Results 65 cases(9.56%)of positive DDH were detected by Graf ultrasound,including 43 cases of dysplasia of hip joint,14 cases of subluxation of hip joint and 8 cases of total dislocation.There were 45 cases(6.62%)of positive DDH by X-ray examination,including 26 cases of dysplasia of hip joint,14 cases of subluxation of hip joint and 5 cases of total dislocation.The positive rate was statistically significant(P<0.05).17 cases of DDH dysplasia and 3 cases of DDH total dislocation were omitted by X-ray diagnosis,which were detected as the definite diagnosis through the Graf ultrasound and X-ray reexamination one month later.Conclusion In the early hip joint abnormal examination of the newborns,the application of Graf ultrasonic examination can better observe the section structure of the hip joint.Compared with X-ray examination,it can reduce the missed diagnosis and provide effective evidence and reference for DDH.
