摘要
目的了解综合性医院重症监护病房(intensive care unit,ICU)光环境及其对患者的影响现状,为优化医疗物理环境提供依据。方法自行设计包含测量时间、场地、床单位布置情况和灯具类型数量等内容的采光照明情况表用于收集ICU光环境的一般情况,采用符合国际标准的照度计对3所综合性ICU的光环境现状进行监测。256名患者根据入院顺序编号,应用随机数字表分组实证研究不同光照度对其生理及心理的影响。结果 ICU不同类型床单位布局的照度平均值均大于0.60,其中半开放单人间、半开放双人间、开放式多人间、独立式单人间的日常照度值分别为(244.7±27.9)lx、(239.9±14.3)lx、(246.6±11.7)lx、(258.7±25.6)lx;且不同布局类型的床单位在日常灯源全开(F=3.470,P=0.02)、灯源半开(F=15.602,P<0.01)及灯源关闭(F=14.004,P<0.01)时照度值存在差异。不同光环境下ICU患者的外周血氧饱和度(pulse oxygen saturation,SPO_2)(t=3.557,P<0.01)、满意度(t=2.684,P<0.05)、睡眠质量(t=2.739,P<0.05)差异有统计学意义。结论综合性医院ICU的光照度水平低于国际和国家规定的ICU照度值,光环境质量对患者的生理和心理水平存在影响,提高光环境质量是ICU日常管理中不应忽视的问题。
Objective The objective of this study was to investigate ambient lighting levels in ICU and its effect on patients. Methods A self-designed illumination table including measurement time, site, bed unit layout and number of lamp types was used to collect general information of the lighting environment in ICU. The status of light environment in three integrated ICUs was monitored by illuminometers in accordance with international standards. Meanwhile, 256 patients were numbered according to the order of admission. Random number table was used to study the effects of different illumination on their physiology and psychology. Results The illuminance of ICU for all different types of bed layout was greater than 0.60. Light levels in semi-open single room, semi-open double room, open multi-room and independent single room were (244.7±27.9) lx,(239.9±14.3) lx,(246.6±11.7) lx, and (258.7±25.6) lx respectively. The illumination values of beds with different layout types were different when the light source was fully open (F=3.470, P=0.02), half-open (F=15.602, P<0.01) and closed (F=14.004, P<0.01). There was significant difference in pulse oxygen saturation (SPO2)(t=3.557, P<0.01), patient satisfaction (t=2.684, P<0.05) and sleep quality (t=2.739, P<0.05) among ICU patients under different light conditions. Conclusions Ambient lighting levels of ICU in general hospitals is lower than that of ICU prescribed by international and national standards. The quality of light environment has an impact on the physiological and psychological level of patients. Improving the quality of light environment should not be neglected in the daily management of ICU.
