摘要
目的探讨血清胃蛋白酶原(PG)Ⅰ、PGⅡ、miRNA-21、let-7a检测对胃癌的诊断价值。方法选择75例胃癌患者(研究组)和同期60例健康体检者(对照组),应用全自动化学发光分析仪检测两组受试者的血清PGⅠ、PGⅡ水平,实时荧光定量逆转录聚合酶链反应(q RT-PCR)检测血清miRNA-21、let-7a水平,分析血清PGⅠ、PGⅡ、miRNA-21、let-7a检测对胃癌的诊断价值。结果研究组的血清PGⅠ、let-7a水平及PGⅠ/PGⅡ均明显低于对照组,血清miRNA-21水平明显高于对照组,差异均有统计学意义(P﹤0.01)。不同TNM分期胃癌患者的血清PGⅠ、miRNA-21水平和PGⅠ/PGⅡ比较,差异均有统计学意义(P﹤0.01)。PGⅠ、PGⅠ/PGⅡ、miRNA-21、let-7a诊断胃癌的受试者工作特征(ROC)曲线下面积(AUC)分别为0.884、0.786、0.803、0.805。PGⅠ、PGⅠ/PGⅡ、miRNA-21、let-7a以及PGⅠ联合miRNA-21诊断早期(Ⅰ~Ⅱ期)胃癌的AUC分别为0.883、0.782、0.776、0.780和0.904,其中PGⅠ联合miRNA-21检测的诊断效能最高。Logistic多因素分析结果显示,血清PGⅠ和miRNA-21是早期胃癌发生的独立影响因素(OR=1.53、1.43,P﹤0.05)。结论 PGⅠ、PGⅡ、miRNA-21、let-7a等生物标志物在胃癌患者的血清中异常表达,PGⅠ对早期胃癌的诊断效能较高,其与血清miRNA-21联合检测可提高早期胃癌的诊断效能。
Objective To investigate the diagnostic value of serum PG Ⅰ, PG Ⅱ, miRNA-21 and let-7a for gastric cancer. Method There were 75 patients with gastric cancer in the study group and 60 healthy subjects in the control group. The serum levels of PG Ⅰ and PG Ⅱ were determined by automatic chemical analyzer. The serum levels of miRNA-21 and let-7a were detected by quantitative reverse transcription polymerase chain reaction (qRT-PCR). The diagnostic value of PG Ⅰ, PG Ⅱ, miRNA-21 and let-7a for gastric cancer were analyzed. Result Compared with the control group, the levels of PG Ⅰ and let-7a and PG Ⅰ/PG Ⅱ in the study group were significantly lower while the serum level of miRNA-21 in the study group was significantly higher (P<0.01). There were significant differences in serum levels of PG Ⅰ, miRNA-21 and PG Ⅰ/PG Ⅱ between patients with different TNM staging (P<0.01). The areas under ROC curve (AUC) of PG Ⅰ, PG Ⅰ/PG Ⅱ, miRNA-21, let-7a for diagnosis of gastric cancer were 0.884, 0.786, 0.803, 0.805, respectively. The AUC of PG Ⅰ, PG Ⅰ/PG Ⅱ, miRNA-21, let-7a and PG Ⅰ combined with miRNA-21 in the diagnosis of early gastric cancer (stage Ⅰ-Ⅱ) were 0.883, 0.782, 0.776, 0.780 and 0.904, respectively. PG Ⅰ combined with miRNA-21 had the highest diagnostic efficiency. The incidence of early gastric cancer was correlated with serum level of PG Ⅰ(OR=1.53, P<0.05) and miRNA-21 (OR= 1.43, P<0.05) calculated by Logistic regression analysis. Conclusion The biomarkers such as PG Ⅰ, PG Ⅱ, miRNA-21 and let-7a in patients with gastric cancer are expressed abnormally in serum. Diagnostic efficacy of PG Ⅰ on early gastric cancer is high, which can be improved by the combined analysis with serum level of miRNA-21.
