PICC与传统经锁骨下静脉、股静脉途径穿刺置管在血液病患者静脉化疗中的应用比较 被引量:29

摘要 目的探讨外周静脉穿刺的中心静脉置管(PICC)与传统经锁骨下静脉、股静脉途径穿刺置管三种不同方法在血液病患者静脉化疗中的应用。方法回顾性分析2015年1月至2017年1月进行静脉化疗的血液病患者173例,其中PICC组58例、锁骨下静脉组63例和股静脉组52例。观察三种途径在置管的成功率、导管留置时间及主要相关并发症发生率的差异。结果锁骨下静脉组一次置管成功率明显低于PICC组与股静脉组(P<0.05)。PICC组导管平均留置时间明显高于锁骨下静脉组与股静脉组,差异有统计学意义(P<0.01)。股静脉组总并发症发生率明显高于PICC组与锁骨下静脉组,差异有统计学意义(P<0.05);PICC组与锁骨下静脉组总并发症发生率差异无统计学意义(P>0.05)。结论对血液病患者行PICC置管化疗,具有操作简单、留管时间长、并发症少等特点,且可减少化疗药外渗的发生率,为血液病患者治疗提供了一条理想的静脉通道。 Objective To compare the application effects of peripherally inserted central catheterization(PICC)and conventional subclavian vein catheterization or femoral vein catheterization in intravenous chemotherapy for patients with blood diseases.Methods The clinical data about 173 patients with blood diseases who were treated by intravenous chemotherapy in our hospital from January 2013 to January 2015 were retrospectively analyzed.These patients were divided into three groups,PICC group(n=58),subclavian vein catheterization(SVC)group(n=63)and femoral vein catheterization(FVC)group(n=52).The success rate of catheterization,the average retention time and incidence of complications were observed and compared among the three groups.Results The one-time catheterization success rate in SVC group was significantly lower than that in PICC group and FVC group(P<0.05).The average retention time in PICC group was significantly longer than that in SVC group and FVC group(P<0.01).Moreover the incidence of complications in FVC group was significantly higher than that in PICC group and SVC group(P<0.05),however there was no significant difference between PICC group and SVC group(P>0.05).Conclusion PICC catheterization is a simple,safe and effective method in intravenous chemotherapy for patients with blood diseases,with less complications and lower incidence rate of drug exosmose,and it is an ideal method in intravenous chemotherapy of patients with blood diseases.
