摘要
目的建立符合现阶段临床需求的先天性心脏病(CHD)患儿术后康复需求的呼吸道管理方案,探讨该方案的适用价值。方法纳入2016年1月至2017年5月于我院接受手术治疗的72例CHD患儿为对象,按照随机数字表法分为两组各36例。对照组常规开展呼吸道管理,观察组应用量化评估模式指导呼吸道管理。比较两组患儿术后呼吸系统相关并发症发生情况、术后恢复效果,于首次吸痰时观察两组血氧饱和度、呼吸频率、心率变化等情况。结果两组术后呼吸系统相关并发症发生率差异无统计学意义(P> 0. 05)。观察组机械通气时间、术后监护时间、术后住院时间均明显短于对照组(P <0. 05),吸痰前血氧饱和度明显高于对照组,呼吸频率明显更低,心率明显更低,差异有统计学意义(P <0. 05);两组吸痰后5min上述指标差异均无统计学意义(P> 0. 05)。结论 CHD患儿术后易出现呼吸系统相关并发症,开展合理的呼吸道管理是必要的;以量化评估模式指导呼吸道管理,能够更有效减少并发症、促进患儿术后康复。
Objective To establish a respiratory management program that should meet current clinical needs for children with congenital heart disease(CHD)and to explore the practical value of the program.Methods Seventy-two children with CHD treated in our hospital from January 2016 to May 2017 were randomly divided into control or observation group,36 in each group.The control group was given routine respiratory tract management,and the observation group was nursed according to quantitative evaluation model.The incidence of complications related to respiratory system and postoperative recovery were compared between the two groups.The changes of blood oxygen saturation,respiratory rate and heart rate during first time of sputum suction were observed.Results There were no significant difference in the incidence of complications related to respiratory system between the two groups(P>0.05).The mechanical ventilation time,postoperative monitoring time and postoperative hospital stay in the observation group were significantly shorter than those in the control group(P<0.05).Before first time of sputum suction,blood oxygen saturation was significantly higher while respiratory rate and heart rate were significantly lower in the observation group than those in the control group(P<0.05).However,these indexes did not show significant between the two groups after 5 minutes of sputum suction(P>0.05).Conclusion For children with congenital heart disease after operation,complications related to respiratory system are common.Respiratory management is necessary and reasonable.The quantitative evaluation model to guide the management of respiratory tract can effectively reduce complications and promote the rehabilitation of children after operation.
