摘要
目的了解湘西地区城乡居民对分级诊疗制度的知晓情况及影响因素,为发展完善分级诊疗制度提供参考。方法 2018年1～7月,在湘西地区随机抽取1265名居民,采取自制封闭式调查问卷进行面对面访谈形式的调查,回收有效问卷1208份,并用Excel 2003进行数据录入,利用图表对数据进行分析。结果城市居民的知晓率为47.0%,乡镇居民的知晓率为21.7%。城市居民主要通过社区卫生服务机构宣传了解分级诊疗制度(35.1%),乡镇居民主要通过医生介绍了解分级诊疗制度(41.6%)。居民认为基层医院服务能力不足是影响分级诊疗制度发展的主要因素。结论湘西地区城乡居民分级诊疗制度知晓率有待提高,城乡居民知晓率差距大,城乡居民了解分级诊疗制度的途径有所差异,各级相关机构应加大宣传力度,推动居民了解分级诊疗制度,缩小城乡居民知晓率差距,促进该制度有效实施。
Objective To understand the knowledge and influencing factors of classified diagnosis and treatment system in urban and rural residents in western Hu′nan,and to provide reference for the development and improvement of classified diagnosis and treatment system. Methods From January to July 2018,1265 residents were randomly selected in western Hu'nan for face-to-face interview with self-made closed questionnaire,and 1208 valid questionnaires were collected. The data were recorded by Excel 2003 and analyzed with charts. Results The awareness rate of urban residents was 47.0% and that of township residents was 21.7%. Urban residents understood the classified diagnosis and treatment system mainly through the promotion of community health service organizations (35.1%),and township residents understood the classified diagnosis and treatment system mainly through the introduction of doctors (41.6%). Residents believed that the lack of service capacity of primary hospitals was the main factor affecting the development of classified diagnosis and treatment system. Conclusion The awareness rate of the classified diagnosis and treatment system in urban and rural residents in western Hu'nan needs to be improved,and the gap of the awareness rate between urban and rural residents is wide. The ways for urban and rural residents to understand the classified diagnosis and treatment system are different. The relevant institutions at all levels should increase their publicity efforts to promote residents to understand the classified diagnosis and treatment system,reduce the gap between urban and rural residents,and promote the effective implementation of the system.
