摘要
目的对比分析股骨近端防旋髓内钉(PFNA)与人工股骨头置换术(FHR)治疗股骨粗隆间骨折的效果。方法选取我院2017年1月~2018年4月收治的80例股骨粗隆间骨折患者为研究对象,以随机数字表法分为PFNA组和FHR组,每组各40例,比较两组患者关节恢复优良率,手术时间、术中出血量、骨折愈合时间等临床指标以及并发症的发生情况,采用髋关节功能Harris评分评价两组患者手术前后关节功能情况。结果 PFNA组术后恢复优良率为85.00%,FHR组术后恢复优良率为62.50%,差异有统计学意义(P <0.05);PFNA组的手术时间、术中出血量、骨折愈合时间均短于FHR组,差异有统计学意义(P <0.05);手术后PFNA组的Harris评分高于对照组,差异有统计学意义(P <0.05);两组的并发症发生率比较,差异无统计学意义(P> 0.05)。结论较人工股骨头置换术而言,PFNA治疗股骨粗隆间骨折能够显著改善术后关节功能的恢复情况,缩短手术时间与骨折愈合时间、减少术中出血量,利于患者康复,且对并发症未有显著影响。
Objective To compare and analyze the effects of proximal femoral nail anti-rotation(PFNA) and artificial femoral head replacement(FHR) in the treatment of femoral intertrochanteric fractures. Methods 80 patients with femoral intertrochanteric fractures admitted to our hospital from January 2017 to April 2018 were selected and divided into PFNA group and FHR group according to the random number table method,with 40 cases in each group.The excellent and good rate of joint recovery,operation time,intraoperative blood loss,fracture healing time and other clinical indicators as well as the incidence of complications were compared between the two groups.Harris score of hip joint function was used to evaluate the joint function of the two groups before and after surgery. Results The good rate of postoperative recovery was 85.00% in PFNA group and 62.50% in FHR group(P < 0.05).The operative time, intraoperative blood loss and fracture healing time in PFNA group were shorter than those in FHR group(P < 0.05).The Harris score in PFNA group was higher than that in control group after operation(P < 0.05).There was no statistical difference in the incidence rate of complications between the two groups(P > 0.05). Conclusion Compared with artificial femoral head replacement,PFNA for femoral intertrochanteric fractures can significantly improve the recovery of postoperative joint function,shorten the operative time and fracture healing time,reduce the intraoperative blood loss,help patients’ recovery,and it has no significant impact on complications.
