摘要
目的:明确秦皮汤内服联合低温超声雾化治疗单纯疱疹病毒性角膜炎的疗效。方法:选取至本院眼科就诊的单纯疱疹病毒性角膜炎患者共76例132眼,根据随机原则分为治疗组38例68眼、对照组38例64眼。治疗组给予秦皮汤300 m L分为每日两次口服、共4周,秦皮汤浓煎药液眼部低温超声雾化治疗,每次20 min、隔日一次、共4周,0. 1%更昔洛韦眼水每2 h一次、0. 5%更昔洛韦眼胶每晚一次点患眼;对照组仅予浓度、频次相等的更昔洛韦眼水、眼胶等局部治疗;治疗前、治疗后观察患者角膜荧光素染色(fluorescence,FL)评分、角膜知觉值、中医证候评分、总有效率及治愈时间,随访并计算一年内的复发率。结果:治疗前两组患者的FL评分、角膜知觉值、中医证候评分均无明显差异(P>0. 05);治疗后2周、4周两组均观察到FL评分和中医证候评分降低、角膜知觉值上升(P <0. 05),且治疗组指标的改善程度明显高于对照组(P <0. 05);治疗组治愈52眼(治愈率76. 5%)、总有效率97. 1%,对照组治愈33眼(治愈率51. 6%)、总有效率87. 5%,治疗组总有效率高于对照组(χ2=4. 302,P <0. 05)、平均治愈天数较对照组明显缩短(P <0. 05);治疗组复发率(7. 4%)明显低于对照组(20. 3%)(χ2=4. 702,P <0. 05)。结论:与单纯西药局部治疗相比,秦皮汤内服联合低温超声雾化的中西医结合方法治疗单纯疱疹病毒性角膜炎的疗效更为显著。
Objective: To evaluate the thrapeutic effects of Qinpi Decoction combined with low temperature ultrasonic atomization on herpes simplex keratitis( HSK). Methods: Seventy-six patients( 132 eyes) were randomly divided into two groups: 38 patients( 68 eyes) in therapy group and 38 patients( 64 eyes) in control group. Patients in therapy group were treated with Qinpi Decoction combined with low temperature ultrasonic atomization for 4 weeks,0. 1% ganciclovir eye drops Q2 H,0. 5% ganciclovir gel QN. Meanwhile,patients in control group were only treated with the same concentration of ganciclovir eye drops and gel. Fluorescence( FL) scores,corneal sensitivity,traditional Chinese medicine( TCM) syndrome scores,total therapeutic efficiency and recurrence rate in one year were observed. Results: There was no significant difference in FL scores,cornea sensitivity,TCM syndrome scores between groups before therapy( P > 0. 05). FL scores and TCM syndrome scores were decreased and cornea sensitivity values were increased in two groups at 2 and 4 weeks( P < 0. 05) and improvments were superior in the therapy group( P <0. 05). Total therapeutic efficiency of the therapy group( 97. 1%) was much more higher than the control group’ s( 87. 5%)(χ2= 4. 302,P < 0. 05) and average cure time in the therapy group( 10. 75 ± 3. 59 days) was shorter than that of the control group( 16. 81 ± 5. 12 days)( P < 0. 05). The recurrence rate was 7. 4% in the therapy group and 20. 3% in the control group,respectively(χ2= 4. 702,P < 0. 05). Conclusion: The therapeutic effects of Qinpi Decoction combined with low temperature ultrasonic atomization on HSK were more remarkble than topical administration of antiviral medicine only.
