摘要
目的探讨缺铁性贫血(IDA)对婴幼儿智力发育的影响。方法选择2017年1月至2018年8月在乐清市人民医院儿童保健门诊行健康检查的870例6月龄至3岁婴幼儿为研究对象,均给予血常规检查,比较不同月龄段小儿贫血的检出率,采用Bayley婴幼儿智力量表行智力发育指数、运动发育指数的测评。结果 6~11月龄儿童IDA的检出率为16.09%(65/404),12~23月龄和24~36月龄组IDA的检出率分别为4.07%(10/246)和1.36%(3/220),随年龄增长, IDA检出率逐渐下降(χ~2=43.31,P<0.05);男童和女童IDA的检出率无显著性差异(χ~2=0.09,P>0.05);IDA患儿智能发育指数(MDI)和运动发育指数(PDI)分别为(97.25±7.84)和(104.36±9.18)分,明显低于无缺铁性贫血儿童的(109.22±8.10)和(112.25±10.03)分(均P<0.05)。结论 6月龄至3周岁儿童IDA随年龄增长,其患病率逐渐下降,IDA影响婴幼儿智能行为发育,应积极预防纠正。
Objective To investigate the influence of iron deficiency anemia (IDA) on intelligence of infants.Methods From January 2017 to August 2018,870 infants aged 6 months to 3 years who took healthy care examination in People’s Hospital of Yueqing were selected.Blood routine examination was detected,and detection rate of IDA was compared among infants of different age.Bayley Infant Intelligence and Motion Scale was used to evaluate the development of children's intelligence and physical development.Results The detection rate of IDA was 16.09%(65/404) for 6 to 11 month-old infants,and that was 4.07%(10/246) and 1.36%(3/220) for infants aged 12-23 months and aged 24-36 months,respectively.IDA detection rate decreased with increasing of age (χ^2=43.31, P <0.05).There was no significant difference in the detection rate between boys and girls (χ^2=0.09,both P >0.05).The mental development index (MDI) and physical development index (PDI) of IDA infants were 97.25±7.84 and 104.36±9.18,respectively,which were significantly lower than those of infants without nutritional iron deficiency anemia ( 109.22± 8.10,112.25±10.03)(both P <0.05).Conclusion The prevalence of IDA in infants aged 6 months to 3 years decreases with age increasing.IDA affects the development of infants' intelligence and behavior,so it should be actively prevented and corrected.
