摘要
目的探讨男性非酒精性脂肪性肝病(non-alcoholic fatty liver disease,NAFLD)患者RPR指数与肝纤维化的相关性。方法选取2013年1月至2017年12月延安大学附属医院门诊NAFLD男性患者180例,健康对照者男性60名,进行血小板计数(PLT)、红细胞分布宽度(RDW)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、FibroScan检测,同时进行肝纤维化BARD评分、FibroScan检测分期,对不同组检测及评分结果进行相关统计学分析。结果男性NAFLD患者与健康对照者RPR指数差异有统计学意义(P<0.05);不同BARD评分组男性NAFLD患者RPR指数差异有统计学意义(P<0.05);男性NAFLD患者RPR指数与FibroScan的检测值呈正相关;不同FibroScan肝纤维化分期男性NAFLD患者RPR指数差异有统计学意义(P<0.05)。结论男性NAFLD患者RPR指数与肝脏肝纤维化存在相关性,随着肝纤维化进展,RPR指数有升高的趋势,监测RPR指数的变化有利于更好、更便捷地动态监测脂肪肝患者肝组织纤维化的进展,协助早期诊断NAFLD肝纤维化发生,及时制定有效逆转肝纤维化的治疗方案,改善脂肪肝预后和转归。
Objective To explore the relationship between RPR and hepatic fibrosis in male patients with non-alcoholic fatty liver disease(NAFLD).Methods One hundred and eighty male outpatients with NAFLD and 60 healthy male controls in Yan’an University Affiliated Hospital were selected.The platelet count(PLT),red blood cell distribution width(RDW),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and FibroScan value were tested,in the meantime,the BARD scores and the liver FibroScan stages were made,then the results of testing and scoring were studied retrospectively.Results There was statistical significance of RPR index between the male with NAFLD and the healthy controls(P<0.05).There was statistical significance of RPR index in the different BARD score groups(P<0.05).The RPR index was positively correlated with FibroScan value in the male with NAFLD.There was statistical significance of RPR index in the different FibroScan stages groups(P<0.05).Conclusion There is correlation between the RPR index and hepatic fibrosis in male NAFLD,the RPR index increases with the progress of liver fibrosis,monitoring the changes of RPR index is conducive to monitor the progress of liver fibrosis,help early diagnosis of non-alcoholic fatty liver fibrosis,make an effective reverse hepatic therapy,and improve the prognosis and outcome of fatty liver.
