胱抑素C对高龄老年心力衰竭患者肾脏损害的诊断价值 被引量:6

摘要 目的 研究胱抑素 C ( Cys -C)检测对高龄老年心力衰竭患者肾脏损害的诊断价值.方法 选取2016 年8 月~2017 年8 月我院收治的150 例高龄老年心力衰竭患者, 根据是否伴有肾脏损害分为心力衰竭伴有肾脏损害组( A组)和心力衰竭无肾脏损害组( B 组) , 选取同期我院收治的150 例无心力衰竭的高龄老年心脏病患者作为对照组, 比较血清Cys-C 水平及相关临床指标.结果 A 组患者血清Cys-C水平、 B型脑钠肽前体(pro-BNP)及肌酐( Cr)明显高于 B 组和对照组( P <0. 05);尿素氮( BUN)和左室射血分数( LVEF)水平均明显低于B组和对照组( P <0. 05);心功能Ⅳ级患者的Cys-C水平、 pro-BNP和LVEF与Ⅱ级和Ⅲ级患者相比差异有统计学意义( P <0. 05) .结论 血清 Cys -C水平可以有效反映心衰患者的心肾功能受损程度, 对心衰患者早期肾脏损害进行预测, 具有较高的诊断价值. Objective To explore the diagnostic potential o£ serum cystatin C ( Cys - C) for renal damage in older patients with heart failure. Methods 150 older patients with congestive heart failure from August 2016 to August 2017 were recruited. The patients were divided into the group A (with renal damage) and the group B ( without renal damage). 150 older patients with heart diseases but without heart failure were selected as the control group. Serum level of Cys - C and clinical indicators of heart and kidney functions were recorded. Results The levels of Cys - C, pro - brain natriuretic peptide ( pro - BNP) and creatinine ( Cr) in the group A were significantly higher than those in the group B and the control group ( P < 0. 05 ). Blood urea nitrogen ( BUN) and LVEF in the group A were significantly lower than those in the group B and the control group ( P < 0. 05 ). There were significant differences in the levels of Cys - C, pro - BNP and LVEF between the patients with class IV heart failure and patients with class Ⅱ and Ⅲ heart failure (P <0. 05). Conclusion Serum Cys - C level can effectively reflect the heart and kidney function in patients with heart fail ure. It can serve as an indicator of pathological renal function.
