卵巢子宫内膜异位囊肿对卵巢储备功能的影响 被引量:3

摘要 目的通过能较准确代表卵巢储备功能的指标AMH,探究卵巢子宫内膜异位囊肿对卵巢储备功能的影响。方法选取2018年1月~2018年7月就诊于保定市第一中心医院的40例腹腔镜手术的卵巢子宫内膜异位囊肿患者为实验组,同期20例卵巢其他良性包块组及同期20例不孕患者作为对照组。并结合实验组患者的年龄将其分为≤25岁12例、26-30岁9例、31-35岁12例、36-39岁7例,研究对比同一年龄组,实验组与对照组AMH水平。通过T检验及方差分析来比较各组间血清AMH的差异,应用pearson相关分析来探索影响血清AMH水平的因素。结果①同一年龄组,实验组术前血清AMH值较两组对照组均低,差异有统计学意义(P<0.05);而卵巢其他良性包块组与不孕对照组差异无统计学意义(P>0.05);②随着年龄增长,血清AMH水平呈逐渐下降趋势;③大小不同的卵巢子宫内膜异位囊肿的AMH水平比较,差异无统计学差异(P>0.05);④双侧卵巢子宫内膜异位囊肿组较单侧卵巢子宫内膜异位囊肿组术前血清AMH值低,差异有统计学意义(P<0.05)。结论卵巢子宫内膜异位囊肿对卵巢储备功能有一定的不良影响,导致卵巢储备功能的降低;最终影响妊娠结局。双侧卵巢子宫内膜异位囊肿对卵巢子宫内膜异位囊肿的影响大于单侧卵巢子宫内膜异位囊肿;卵巢子宫内膜异位囊肿的大小对卵巢储备功能的影响无明显差异。 Objective Study on the effect of ovarian endometriosis cyst on ovarian reserve function by AMH,which can accurately represent ovarian reserve function.Methods From January 2018 to July 2018,40 patients with ovarian endometriosis cyst undergoing laparoscopic surgery in Baoding first Central Hospital were selected as experimental group.20 patients with other benign ovarian cysts and 20 infertile women were used as control group.and they were divided by different ages into 12 cases aging<25 years old.9 cases aging 26-30 years old,12cases aging 31-35years old and 7 cases aging 36-39 years old.Research and comparison were made on AMH level in patients with different ages.Results ①The experimental group had obviously lower AMH level as than the other benign ovarian cysts and the infertility group,and their difference had statistical significance (P<0.05).While the difference between other benign ovarian cysts and infertilities was not significant.②Older age led to gradually reduced levels of serum AMH.③There was no difference in AMH levels in ovarian endometriosis cysts of different sizes.④The preoperative serum AMH level of bilateral ovarian endometriosis cyst group was lower than that of unilateral ovarian endometriosis cyst group,and the difference had statistical significance (P<0.05).Conclusion Ovarian endometriosis cysts have a certain negative effect on ovarian reserve function.Leading to the decrease of ovarian reserve function,which may ultimately affect the outcome of pregnancy.Bilateral ovarian endometriosis cysts have greater effects on ovarian reserve function than unilateral ovarian endometriosis cysts.There is no difference between the size of ovarian endometriosis cyst and ovarian reserve function.
