系统性红斑狼疮患者外周血粘附分子、免疫球蛋白和细胞因子含量检测的临床意义 被引量:12

Clinical significance of detecting adhesion molecules, immunoglobulins and cytokines in peripheral blood of patients with systemic lupus erythematosus

摘要 目的探讨系统性红斑狼疮(SLE)患者进行外周血生化指标(免疫球蛋白、细胞因子、抗体阳性率、黏附细胞)含量检测对疾病临床诊治的意义。方法选取2015年8月至2017年8月收治的78例SLE患者(39例活动期、39例非活动期)及39例正常健康体检人群作为研究对象,比较3组研究对象免疫球蛋白含量、细胞因子含量、自身抗体阳性率、黏附分子含量的差异,并分析其水平变化的临床意义。结果 3组免疫球蛋白含量(IgA、IgM、IgG)、细胞因子含量(IL-17、IL-23、IFN-γ、TNF-α、IL-4、IL-5)、黏附分子含量(Svcam-1、sICAM-1)差异有统计学意义(P<0.05),活动期患者和非活动期组患者免疫球蛋白含量、细胞因子含量、黏附分子含量显著高于对照组患者,且活动期组患者免疫球蛋白含量、细胞因子含量、黏附分子含量显著高于非活动期组患者。3组患者抗RNP、抗Sm、抗Scl-70、抗AnuA、抗His、抗ds-DNA阳性率差异有统计学意义(P<0.05),活动期患者多项抗体阳性率高于非活动组患者。结论免疫球蛋白、细胞因子、抗体阳性率和黏附细胞的水平在SLE患者外周血中表现高表达现象,并且随着病情活动增强而升高,能够间接指示患者的病情的严重程度,具有一定的临床意义。 Objective To investigate the clinical significance of detecting the biochemical indexes including adhesion molecules, immunoglobulins and cytokines in peripheral blood of patients with systemic lupus erythematosus (SLE). Methods A total of patients with SLE who were admitted and treated in our hospital from August 2015 to August 2017 including 39 cases in active period and 39 cases in nonactive period were enrolled in the study, moreover,the other 39 healthy subjects who underwent physicam examination in our hospital were enrolled as control group. of the levels of immunoglobulins,cytokines, autoantibody positive rate,the levels of adhesion molecules were observed and compared among the three groups, moreover,the clinical significance was analyzed. Results There were significant differences in the levels of immunoglobulins including IgA,IgM and IgG,the levels of cytokines including IL-17,IL-23,IFN-γ,TNF-α,IL-4 and IL-5,the levels of adhesion molecules including Svcam 1,sICAM 1 among the three groups ( P <0.05).The levels of immunoglobulins,cytokines and adhesion molecules in activity period group and in nonactivity period group were significantly higher than those in control group,moreover,which in activity period group were significantly higher than those in nonactivity period group. In addition there were significant differences in the positive rates of anti-RNP,anti-Sm,anti-Scl-70,anti-Anua,anti-His and anti-ds -DNA among the three groups ( P <0.05),moreover, which in activity period group were significantly higher than those in nonactivity period group. Conclusion The immunoglobulins,cytokines,antibody positive rates and adherent cell levels are highly expressed in peripheral blood of patients with SLE,which are increased with the disease activity increase,which can indirectly indicate the severity of disease condition and has a certain clinical significance.
