摘要
目的探讨单纯肺部感染与糖尿病合并肺部感染患者的病原菌分布及耐药性。方法选取2016年3月至2018年3月收治的单纯肺部感染患者100例作为单纯组,同期选取糖尿病合并肺部感染患者100例作为合并组,采用痰标本进行细菌学培养鉴定及药敏试验,分析比较2组病原菌分布及耐药性。结果合并组革兰阴性菌有62株(62.00%)、革兰阳性菌有26株(26.00%)、真菌有12株(12.00%),单纯组革兰阴性菌有48株(48.00%)、革兰阳性菌有42株(42.00%)、真菌有10株(10.00%);药敏试验显示,合并组革兰阴性菌(肺炎克雷伯菌、鲍氏不动杆菌、铜绿假单胞菌)、革兰阳性菌(金黄色葡萄球菌、表皮葡萄球菌)比单纯组更易出现耐药性,合并组和单纯组真菌均无耐药菌检出。结论与单纯肺部感染比较,糖尿病合并肺部感染患者病原菌分布以革兰阴性菌(肺炎克雷伯菌、鲍氏不动杆菌、铜绿假单胞菌)为主,且革兰阴性菌、革兰阳性菌均更易出现耐药性,应进行细菌学培养鉴定及药敏试验,以对不同病原菌选用合理的药物进行治疗。
Objective To investigate the distribution and drug resistance of pathogenic bacteria in patients with simple pulmonary infection and diabetes mellitus complicated by pulmonary infection. Methods One hundred patients with simple pulmonary infection who were admitted and treated in our hospital from March 2016 to March 2018 were enrolled as simple infection group,at the same time, the other 100 patients with diabetes mellitus complicated by pulmonary infection were enrolled as combination infection group.The sputum specimens were used for bacteriological culture,identification and drug sensitivity test.The distribution and drug resistance were observed and compared between the two groups. Results There were 62 strains of gram-negative bacteria in combination infection group (62%),26 strains of gram-positive bacteria (26%),and 12 strains of fungi (12%).There were 48 strains of gram-negative bacteria in simple infection group (48%),42 strains of gram-positive bacteria (42%),and 10 strains of fungi (10%).The drug sensitivity test showed that the gram-negative bacteria including Klebsiella pneumoniae,Acinetobacter baumannii,Pseudomonas aeruginosa and gram-positive bacteria including Staphylococcus aureus,Staphylococcus epidermidis in combination infection group were more easy to appear drug resistance than those in simple infection group.Moreover no fungi were detected in both groups. Conclusion As compared with that in patients with simple pulmonary infection,the distribution of pathogenic bacteria in patients with diabetes mellitus complicated by pulmonary infection is mainly gram-negative bacteria including Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa,moreover,the gram negative bacteria and gram-positive bacteria are more easy to appear drug resistance.Bacteriological culture identification and drug sensitivity test should be carried out to select reasonable drugs for different pathogenic bacteria.
