右美托咪定复合罗哌卡因胸椎旁神经阻滞在单侧开胸手术后的镇痛效果分析 被引量:8

摘要 目的分析右美托咪定复合罗哌卡因胸椎旁神经阻滞(TPVB)在单侧开胸手术后的镇痛效果。方法选取本院2017年3月至2018年6月收治的56例单侧开胸食管癌手术患者进行分组研究,按照单双号法分为对照组(n=28)和实验组(n=28),两组均在全麻诱导前实施开胸肋间TPVB,对照组予以罗哌卡因麻醉,实验组予以右美托咪定复合罗哌卡因麻醉,比较两组镇痛效果、安全性。结果实验组与对照组的疼痛阻滞起效时间、阻滞平面固定时间、脊神经阻滞范围、手术持续时间、气管导管拔除时间、出苏醒室时间相比,差异无统计学意义;实验组追加吗啡、追加咪达唑仑、恶心呕吐发生率均显著低于对照组,组间差异有统计学意义(P<0.05)。结论单侧开胸手术后应用右美托咪定复合罗哌卡因胸椎旁神经阻滞镇痛效果良好,安全可行,值得在临床中广泛应用。 Objective To analyze the analgesic effect of dexmedetomidine combined with ropivacaine for thoracic paravertebral nerve block(TPVB) after unilateral thoracotomy. Methods A total of 56 patients with unilateral open thoracic esophageal cancer who underwent surgery from March 2017 to June 2018 in our hospital were enrolled in this study. They were divided into control group(n=28) and experimental group according to single and double numbers(n=28). Both groups underwent thoracic intercostal TPVB before general anesthesia induction. The control group received ropivacaine anesthesia. The experimental group received dexmedetomidine combined with ropivacaine anesthesia. The analgesic effect and safety of the two groups were compared. Sexual differences. Results There was no significant difference in the onset time of pain block, the time of block fixation, the range of spinal nerve block, the duration of surgery, the time of tracheal catheter removal, and the time of the awakening room in the experimental group and the control group;the incidence of additional morphine, additional midazolam, and nausea and vomiting in the experimental group were significantly lower than those in the control group, and there was significant statistical difference between the groups(P<0.05).Conclusion The application of dexmedetomidine combined with ropivacaine for thoracic paraspinal nerve block after unilateral thoracotomy has good analgesic effect, is safe and feasible, and is worthy of wide application in clinical practice.
