摘要
目的探讨妊娠期糖尿病孕妇甲状腺功能异常与甲状腺自身抗体的相关性及其临床应用价值。方法纳入于2015年1月-2016年12月在本院治疗的1 127例妊娠期糖尿病孕妇,应用非参数Spearman等级相关分析对甲状腺功能异常的妊娠期糖尿病孕妇甲状腺功能异常与甲状腺自身抗体的相关性进行分析。结果 1 127例妊娠期糖尿病孕妇中,TPO-Ab、甲状腺球蛋白抗体同时阳性检出率为8.5%(96/1 127),甲状腺自身抗体单独阳性检出率为4.1%(47/1 127),甲状腺球蛋白抗体单独阳性检出率为5.2%(59/1 127)。亚临床型甲状腺功能减退症检出率为26.6%(300/1 127),低甲状腺素血症检出率为1.8%(20/1 127),临床型甲状腺功能减退症检出率为8.4%(95/1 127)。甲状腺功能正常的孕妇中,甲状腺自身抗体阳性检出率为18.9%(143/755),甲状腺球蛋白抗体阳性检出率为20.5%(155/755)。结论妊娠糖尿病孕妇早期甲状腺功能指标检测不能够完全反应甲状腺功能,妊娠糖尿病孕妇早期甲状腺激素与甲状腺自身抗体的联合检测可完全反应妊娠糖尿病孕妇的甲状腺功能,具有重要的临床价值和意义。
Objective To discuss and analyze the correlation as well as its clinical value between thyroid dysfunction and thyroid autoantibodies in pregnant women with gestational diabetes mellitus.Methods A total of 1127 pregnant women with gestational diabetes in our hospital from January 2015 to December 2016 were selected in ihis research,and the correlation between abnormal thyroid function and thyroid autoantibody in pregnant women with gestational diabetes mellitus was analyzed with nonparametric Spearman level correlation analysis.Results Among 1127 pregnant women with gestational diabetes,the positive rate of TPO-Ab and thyroglobulin antibody was 8.5%(96/1 127),the positive rate of thyroid autoantibody was 4.1%(47/1 127),and the positive rate of thyroid globulin antibody was 5.2%(59/1 127).The detection rate of subclinical hypothyroidism was 26.6%(300/1 127),the detection rate of hvpothyroxemia was 1.8%(20/1 127),and the detection rate of bed type hypothyroidism was 8.4%(95/1 127).In the pregnant women with normal thyroid function,the positive rate of thyroid autoantibody was 18.9%(143/755),the positive rate of thyroglobulin antibody was 20.5%(155/755).Conclusion The detection of early thyroid function index in pregnant women with gestational diabetes can't fully respond to thyroid function.The combined detection of thyroid hormone and thyroid autoantibody in pregnant women with gestational diabetes can fully respond to the thyroid function of pregnant women with gestational diabetes,which has important clinical value and significance.
