MRI检查与X线平片对膝关节退行性骨关节病变中的诊断价值对比 被引量:6

摘要 目的:对比MRI检查与X线平片对膝关节退行性骨关节病变的诊断价值。方法:抽取2017年3月~2018年9月收治的86例膝关节退行性骨关节病变患者为研究对象。依据诊断方法的差异,分成对照组(43例)和观察组(43例),分别给予两组X线平片诊断、MRI诊断。结果:观察组诊断符合率(95.35%)高于对照组(81.40%),差异有统计学意义(P<0.05)。关节软骨变性:MRI检查见T1WI的T1值延长,软骨磨损、变薄,软骨连续性不完整,软骨下骨伴骨髓水肿及囊性变;X线平片见于软骨严重褪变时,表现为关节面不光整,关节面下骨质硬化、囊状低密度影。韧带慢性损伤:MRI检查见韧带肿胀、增粗、信号增高,以前交叉韧带较常见,伴有慢性撕裂时表现为部分或全部纤维走行不连续;X线平片不能显示韧带。半月板退变:MRI检查可见半月板肿胀,局部见条状、线状高信号影,严重者半月板向关节腔外移位;X线平片不能直接显示半月板,可显示半月板钙化。关节积液:MRI检查关节囊、关节腔内见T1WI低信号、T2WI高信号影;X线平片见髌上囊、髌下脂肪垫肿胀、密度增高。MRI检查及X线平片均可清晰显示关节游离体、关节缘骨质增生、关节间隙变窄。结论:膝关节退行性骨关节病变诊断中,MRI检查的诊断符合率及骨关节病变检出率均优于X线平片。
