摘要
目的对腱鞘巨细胞瘤的超声表现进行探讨,并评价超声对腱鞘巨细胞瘤的临床诊断价值。方法回顾我院49例经手术病理证实腱鞘巨细胞瘤患者临床资料,通过超声、CT及MRI检查进行综合性分析,从而评价超声在腱鞘巨细胞瘤中的临床诊断价值。结果腱鞘巨细胞瘤超声表现为发生于肌腱周边或关节周边的实质低回声肿块,可单发或多发,边界清晰,形态可规则或不规则,内部回声呈均匀分布,部分可见钙化或液化现象,肿块可有骨骼压迫,或关节囊侵袭,彩色多普勒(CDFI)示肿块内部可见较丰富的血流信号。超声、MRI、CT三种方式在诊断肿块位置、大小、形态、数目、与周围组织关系等方面差异无统计学意义(P>0.05)。结论超声对腱鞘巨细胞瘤诊断准确率较高,相比CT、MRI具有操作简便、经济实惠、安全无辐射的优势,可作为临床诊断腱鞘巨细胞瘤的首选检查方法。
Objective To investigate the ultrasonographic features of giant cell tumor of tendon sheath and evaluate the value of ultrasound in the diagnosis of giant cell tumor of tendon sheath. Methods The clinical data of 49 cases of giant cell tumor of tendon sheath confirmed by operation and pathology in our hospital were analyzed by ultrasound, CT and MRI, so as to evaluate the clinical diagnostic value of ultrasound in the giant cell tumor of tendon sheath. Results The sonographic manifestation of giant cell tumor of the tendon sheath was a hypoechoic mass surrounding the tendon or the periphery of the tendon. It could be single or multiple,with clear boundaries,regular or irregular shape,a homogeneous distribution of internal echoes, some calcification, or the mass of the bone,or the invasion of the joint capsule. CDFI: More abundant blood flow signals were visible inside the mass. Ultrasound,MRI and CT had no significant difference in the location, size, shapes number and relationship with the surrounding tissues in three ways (P>0.05). Conclusion Ultrasound has a high diagnostic accuracy in the giant cell tumor of the tendon sheath. Compared with CT and MRI, it has the advantages of simple operation,economical and safe without radiation. It can be used as the first choice for clinical diagnosis of giant cell tumor of tendon sheath.
