摘要 目的探讨飞秒激光制瓣LASIK治疗屈光不正的临床效果。方法选取2013年12月至2016年12月我院收治的156例(302只眼)屈光不正患者为研究对象,根据手术方式的不同将其分为对照组(78例,151只眼)与观察组(78例,151只眼)。对照组患者实施角膜刀制瓣LASIK进行治疗,观察组患者实施飞秒激光制瓣LASIK进行治疗,对比两组患者的手术制瓣成功率、制瓣厚度、裸眼视力、屈光度、总高阶像差及术后并发症发生情况。结果观察组的制瓣成功率显著高于对照组,且制瓣厚度显著薄于对照组,差异具有统计学意义(P<0.05);术后1、3、6、12个月,观察组的裸眼视力均显著优于对照组,差异具有统计学意义(P<0.05);术后观察组的屈光度及总高阶像差均显著优于对照组,差异具有统计学意义(P<0.05);观察组的手术并发症总发生率显著低于对照组,差异具有统计学意义(P<0.05)。结论飞秒激光制瓣LASIK治疗屈光不正效果显著,可以提高制瓣成功率及患者的裸眼视力,降低制瓣厚度及并发症总发生率,改善患者的术后屈光状态稳定性,减少术源性像差,值得临床推广与应用。 Objective To study the clinical effect of femtosecond laser flap LASIK in the treatment of refractive error.Methods A total of 156 patients(302 eyes) admitted in our hospital from December 2013 to December 2016 were selected as the study objects. The patients were divided into control groups(78 cases, 151 eyes) and observation groups(78 cases, 151 eyes) according to the different surgical methods. The control group were treated with corneal knife flap LASIK, and the observation group was treated with femtosecond laser flap LASIK. The success rates of sungical valvuloplasty, flap thickness, naked vision, diopter, total high order aberration and postoperative complications of the two groups were compared. Results The success rate of surgical valvuloplasty of the observation group was significantly higher than that in the control group, and the flap thickness was significantly thinner than that in the control group, and the differences were statistically significant(P <0.05). At 1, 3, 6, 12 months after operation, the naked vision of the observation group was significantly higher than that of the control group and the differences were statistically significant(P <0.05). After operation, the diopter and total high order aberration of the observation group were superior to those of the control group, and the differences were statistically significant(P<0.05). The total incidence of surgical complications of the observation group was significantly lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion The femtosecond laser flap LASIK has a significant effect in the treatment of refractive error. It can improve the success rate of surgical valvuloplasty and patient’s naked vision, reduce the flap thickness and total incidence of complications, improve postoperative refractive state stability and reduce the surgically induced corneal higher-order aberrations, which is worthy of clinical promotion and application.
