摘要
目的观察血浆生长分化因子-15(GDF-15)、半乳糖凝集素3(Gal-3)和和肽素(CPP)在诊断心肾综合征的临床价值。方法心力衰竭患者206例,根据肾小球滤过率分为单纯心力衰竭组(89例)和心肾综合征组(117例),选择同期值康体检者30例作为对照组用酶联免疫吸附法检测GDF-15,Gal-3和CPP水平。比较对照组,单纯心力衰竭组和心肾综合征组血清GDF-15,Gal-3和CPP水平的比较,心肾综合征组血清GDF-15,Gal-3和CPP水平与心功能,肾功能和预后的相关性,及其各个指标相关性分析。结果心肾综合征组的血清GDF-15,Gal-3和CPP水平明显高于单纯心力衰竭组和对照组(P<0.01),而单纯心力衰竭组的水平明显高于对照组(P<0.01)。心肾综合征患者血清GDF-15,Gal-3和CPP水平随着心功能分级和肾功能分期的升高而升高(P<0.01)。心肾综合征患者血清GDF-15,Gal-3和CPP水平预后不良组明显高于预后良好组(P<0.01)。心肾综合征患者血清GDF-15与Gal-3水平呈正相关(r值分别为0.678、0.719,P<0.05),Gal-3与CPP水平同样也呈正相关(r=0.848,P<0.05)。结论 GDF-15,Gal-3和CPP水平参与了心肾综合征的发生发展,是评价心肾综合征严重程度和预后的重要指标,对临床具有重要的指导意义。
Objective To observe the clinical value of plasma growth differentiation factor15 (GDF15), galectin3 (Gal3) and copeptin (CPP) in diagnosis of cardiorenal syndrome. Methods A total of 206 patients with heart failure were divided into simple heart failure group ( n =89) and cardiorenal syndrome group ( n =117) according to glomerular filtration rate. Thirty healthy physical examinees were taken as controls. The levels of GDF15, Gal3 and CPP of all subjects were measured by ELISA and compared between the three groups. The correlation between the serum levels of GDF15, Gal3, CPP and cardiac function, renal function, prognosis and the correlation between each indexes were analyzed. Results The serum levels of GDF15, Gal3 and CPP of the patients in cardiorenal syndrome group, which increased with the increase of cardiac function and renal function, were significantly higher than those in simple heart failure group and control group ( P <0.01). The serum levels of GDF15, Gal3 and CPP of the patients in simple heart failure group were significantly higher than those in control group ( P <0.01). Moreover, the serum levels of GDF15, Gal3 and CPP of the patients in poor prognosis group were significantly higher than those in good prognosis group ( P <0.01). In addition, the serum levels of GDF15 ( r =0.678, P <0.05) were positively correlated with those of Gal3 ( r =0.719, P <0.05). And the serum levels of Gal3 were positively correlated with those of CPP ( r =0.848, P <0.05). Conclusion The serum levels of GDF15, Gal3 and CPP are involved in the pathogenesis and development of cardiorenal syndrome,which are important indexes in evaluating the condition and prognosis of the disease, which is of great guiding significance in clinical practice.
