摘要
目的:探讨血清碱性磷酸酶(ALP)、乳酸脱氢酶(LDH)、γ谷氨酸转肽酶(GGT)、α-L-岩藻糖苷酶(AFU)、腺苷脱氨酶(ADA)、5'-核苷酸酶(5'-NT)、胆碱酯酶(CHE)、亮氨酸氨基肽酶(LAP)、谷氨酸脱氢酶(GLDH)以及总胆汁酸(TBA)等10项常用肝功能生化指标对原发性肝癌早期诊断的价值。方法:使用全自动生化分析仪对124例原发性肝癌患者(原发性肝癌组)、113例肝良性肿瘤患者(肝良性肿瘤组)和126例健康体检者(对照组)血清中ALP、LDH、GGT、AFU、ADA、5'-NT、CHE、LAP、GLDH及TBA进行检测分析,计算各项指标的受试者工作特征曲线下面积(AUC)、敏感度、特异度等,并与"金标准"甲胎蛋白(AFP)的结果进行比较。结果:原发性肝癌组患者血清ALP、LDH、GGT、AFU、ADA、5'-NT、LAP、GLDH及TBA水平及阳性率均明显高于对照组(P<0.01)和肝良性肿瘤组(P<0.01),CHE水平明显低于正常对照组(P<0.01)和肝良性肿瘤组(P<0.01),阳性率明显高于正常对照组(P<0.01)和肝良性肿瘤组(P<0.01);单项检测诊断原发性肝癌灵敏度最高的是GGT(83.1%),特异度最高的是AFP(93.8%),诊断准确度最高的是AFU(87.9%),AUC最高的是AFP(0.881)。结论:常规肝功能指标及其联合诊断对原发性肝癌的早期诊断具有重要的辅助作用。
Objective:To explore the diagnostic significance in serum enzymes including alkaline phosphatase (ALP),lactate dehydrogenase (LDH),γ-glutamete transpeptidase (GGT),alpha-L-fucosidase(AFU),adenosine deaminase (ADA),5’- nucleotides (5’-NT),cholinesterase (CHE),leucine aminopeptidase (LAP),glutamic acid dehydrogenase (GLDH),and total bile acid (TBA) for primary hepatocarcinoma (PHC).Methods:Automatic biochemical analyzer was used to analyze serum ALP,LDH,GGT,AFU,ADA,5’-NT,CHE,LAP,GLDH and TBA of 124 patients with primary hepatocarcinoma (PHC group),113 patients with benign liver tumors (benign liver tumor group) and 126 healthy subjects (control group).And the area under the receiver operating characteristic curve (AUC),sensitivity,specificity,etc,were calculated for each indicator and compared with the results of the“gold standard”alpha fetoprotein (AFP).Results:The levels and the positive rates in serum ALP,LDH,GGT,AFU,ADA,5’-NT,LAP,GLDH and TBA of PHC patients were significantly higher than those of normal controls ( P <0.01) and benign liver tumor group( P <0.01).The level in serum CHE was significantly lower than the other two groups ( P <0.01),and positive rate was significantly higher ( P <0.01).The highest sensitivity for the diagnosis of PHC was GGT (83.1%),the highest specificity was AFP (93.8%),the highest diagnostic accuracy was AFU (87.9%),and the highest AUC was AFP (0.881).Conclusion:Routine liver function indexes and the combined diagnosis play an important assistant role in the early diagnosis of primary hepatocarcinoma.
