摘要
目的观察金水宝胶囊联合高通量血液透析对糖尿病肾病血液透析患者预后、氧化应激反应和营养状况的影响。方法将86例糖尿病肾病血液透析患者随机分为2组,对照组43例应用高通量血液透析治疗,研究组43例应用金水宝胶囊联合高通量血液透析治疗,2组均持续治疗3个月。记录2组治疗后心血管事件发生情况和随访1年患者生存情况,检测2组治疗前后氧化应激反应指标和营养状况。结果研究组治疗后心血管事件发生率显著低于对照组(P<0.05),随访1年生存率显著高于对照组(P<0.05);2组治疗后血清丙二醛(MDA)水平均显著低于治疗前(P均<0.05),血清谷胱甘肽过氧化物酶(GSH-Px)和过氧化物歧化酶(SOD)水平均显著高于治疗前(P均<0.05),且研究组血清MDA水平显著低于对照组(P<0.05)、血清GSH-Px和SOD水平均显著高于对照组(P均<0.05);2组治疗后血清前白蛋白(PA)、血总蛋白(TP)、血清白蛋白(ALB)、体质量指数(BMI)和上臂肌围(MAMC)水平均显著高于治疗前(P均<0.05),且研究组治疗后PA、TP、ALB水平均显著高于对照组(P均<0.05),2组治疗后BMI和MAMC水平相比差异均无统计学意义(P均>0.05)。结论金水宝胶囊联合高通量血液透析治疗糖尿病肾病血液透析患者能明显改善预后,减轻氧化应激反应,调节营养状况。
Objective It is to observe the effects of Jinshuibao capsule combined with high-throughput hemodialysis on prognosis, oxidative stress and nutritional status of hemodialysis patients with diabetic nephropathy. Methods Eighty-six patients with diabetic nephropathy were randomly divided into two groups. The control group ( n =43) was treated with high-flux hemodialysis. The study group ( n =43) was treated with Jinshuibao capsule combined with high-flux hemodialysis. Both groups were continuly treated for 3 months. The incidence of cardiovascular events and the survival of patients after 1 year of follow-up were recorded. The oxidative stress response indicators and nutritional status of the 2 groups before and after treatment were measured. Results The incidence of cardiovascular events in the study group was significantly lower while the 1-year survival rate was significantly higher than that in the control group ( P <0.05). The levels of serum malondialdehyde (MDA) in the two groups after treatment were significantly lower and that of serum glutathione peroxidase (GSH-Px) and superoxide dismutase (SOD) were significantly higher than those before treatment ( P <0.05), and the level of serum MDA was significantly lower, that of serum GSH-Px and SOD levels were significantly higher in the study group than those in the control group ( P <0.05). The levels of serum prealbumin (PA), total blood protein (TP), albumin (ALB), body mass index (BMI) and upper arm muscle circumference (MAMC) after treatment in the 2 groups were significantly higher than those before treatment ( P <0.05), and the levels of PA, TP in the study group were significantly higher than those in the control group ( P <0.05). There was no significant difference in BMI and MAMC between the two groups ( P >0.05). Conclusion Jinshuibao capsule combined with high-flux hemodialysis can significantly improve the prognosis, reduce oxidative stress and regulate nutritional status in the treatment of diabetic nephropathy hemodialysis patients.
