摘要
目的探讨老年高血压病患者不同服药时间对血压昼夜节律变化的影响,方法选取老年高血压患者100例,根据服药状况随机分为治疗组54例和对照组46例。治疗组在护理干预的情况下晨服苯磺酸氨氯地平,晚服马来酸依那普利;对照组仅晨服苯磺酸氨氯地平和马来酸依那普利,连续治疗8周。结果治疗前,两组患者平均收缩压及平均动脉压各项指标比较,差异均无统计学意义(P>0.05);治疗后,治疗组患者24hSBP、nSBP、dDBP及nDBP下降幅度优于对照组(P<0.05);治疗组患者治疗后非杓型向杓型模式转化例数差异有统计学意义(P<0.05)。结论通过护理干预不同时间服药均能调节高血压患者血压昼夜节律变化,但晨服与晚服结合对血压昼夜节律的调节更有效,抑制晨峰现象且利于血压类型的转变,值得临床推广应用。
Objective To explore the effect of different drug administration time on blood pressure circadian rhythm in elderly patients with hypertension. Methods A total of 100 elderly patients with hypertension were randomly divided into treatment group (54 cases) and control group (46 cases) according to their medication status. The treatment group received amlodipine besylate in the morning and enalapril maleate in the evening under the condition of nursing intervention. The control group was treated with amlodipine besylate and enalapril maleate for 8 weeks. Results Before treatment, there were no significant differences in mean systolic blood pressure and mean arterial pressure between the two groups ( P>0. 05 ). After treatment, the reduction of 24 hSBP, nSBP , dDBP and nDBP in the treatment group was better than that in the control group ( P<0. 05 ). There were statistically significant differences in the number of patients with non-aiytenoid mode changing to arytenoid mode after treatment in the treatment group ( P<0. 05 ). Concl sion Nursing intervention at different times can regulate blood pressure circadian rhythm changes in patients with hypertension ,but the combination of morning and evening gowns is more effective in regulating blood pressure circadian rhythm, inhibiting the morning peak phenomenon and conducive to the transformation of blood pressure type, which is worthy of clinical promotion and application.
