摘要
目的通过测量髋臼软骨覆盖股骨头的间质形态来预测GrafⅡa型发育性髋关节发育不良的转归。方法回顾分析2015年4月至2017年12月在西安交通大学附属红会医院门诊行超声检查的174例6周龄婴幼儿的髋关节超声影像,基于超声影像测量髋臼软骨角(A角)来评价髋臼对股骨头的覆盖程度,并预测髋臼的发育程度。结果髋关节转归:GrafⅡa(+)型134例,其中121例转变为Ⅰ型,13例转变为Ⅱb型;GrafⅡa(-)型40例,其中35例转变为Ⅰ型,5例转变为Ⅱb型。末次随访转归为正常髋关节的GrafⅡa型髋关节的A角均>75°。结论在6周龄时测量A角可加强对髋臼发育的预测,并评估股骨头的软骨覆盖情况,A角>75°可作为髋关节正常发育的诊断标准。
Objective To predict the outcome of abnormalities of the GrafⅡa type hip joint by measuring the interstitial morphology of the femoral head covered by acetabular cartilage. MethodsThe ultrasonic images of hip of 174 infants (6-week-old children) were analyzed retrospectively in Department of Pediatric Orthopaedics, Xi'an Honghui Hospital between April 2015 and December 2017. The acetabular cartilage angle (A angle) was measured to evaluate the acetabular cover condition on the femoral head and to predict the degree of acetabulum development. ResultsThe hip joints outcomes: 134 cases of GrafⅡa (+) type, of which 121 cases were changed to typeⅠ, 13 cases were changed to typeⅡb, and 40 cases of GrafⅡa (-) type, of which 35 cases were changed to type Ⅰ, 5 cases were changed to typeⅡb. At the final follow-up, A angle of the GrafⅡa type hip joint turnning to the normal hip joint was more than 75°. ConclusionAt 6 weeks of age, improving the prediction of acetabular development, and evaluating the cartilage coverage of the femoral head by measuring the A angle,and the A angle>75° can be used as a diagnostic criterion for normal development of the hip joint.
