老年髋部骨折患者几种术后风险预测方法的应用比较 被引量:7

Comparison of several postoperative risk prediction methods in elderly patients with hip fracture

摘要 老年髋部骨折患者的问题日益突出,此类患者常合并一种或多种基础疾病,使手术风险大为增加,目前老年髋部骨折手术转归及预后的评估尚无统一标准。文章围绕用于老年髋部骨折患者术后风险的评估系统进行综述,着重介绍ASA分级、生理学和手术严重性评分系统(Physiological and Operative Severity Score for the Enumeration of Mortality and Morbidity,POSSUM)及改良POSSUM(Portsmouth POSSUM,P-POSSUM)评分系统、诺丁汉姆评分(Nottingham Hip Fracture Score,NHFS)、大坪骨科老年患者手术风险评分系统(Daping Orthopedics Operative Risk Scoring System for Senile Patient,DORSSSP)等8种方法在老年髋部骨折评估中的应用,比较其在髋部骨折领域的应用价值。上述8种方法预测术后风险均有一定效力,因侧重点不同各有优势。DORSSSP系统较其他评分系统更为合适国人,但仍需应一步完善验证。 The problem of hip fracture in the elderly is becoming more and more serious. These patients also present one or more basic diseases, which greatly increase the risk of operation. Currently, there is no uniform standard to analyze the outcome and prognosis of elderly patients with hip fractures. This article reviews the postoperative risk assessment system for elderly patients with hip fracture, and discuss the application of the American Society of Anesthesiologists (ASA) classification, the Physiological and Operative Severity Score for the Enumeration of Mortality and Morbidity (POSSUM) score and modified POSSUM score system, Nottingham Hip Fracture Score (NHFS), Daping Orthopedics Operative Risk Scoring System for Senile Patient (DORSSSP) in the evaluation of hip fracture in elderly patients. Each of the above methods is effective in predicting postoperative risk to various extents, with different advantages. The DORSSSP system is more suitable for Chinese patients than other scoring systems, but it still needs to be verified in further studies.
