摘要
目的探讨高强度聚焦超声联合左炔诺孕酮宫内缓释系统对子宫腺肌病患者血清糖类抗原125(carbohydrate antigen 125,CA 125)、抗子宫内膜抗体(anti endometrial antibody,EMAb)、血红蛋白(hemoglobin,HGB)及卵巢功能的影响。方法选取2015年1月至2017年10月威海市妇幼保健院收治的90例子宫腺肌病患者,按照随机数字表法分为观察组和对照组,每组各45例。对照组采用高强度聚焦超声治疗,观察组在对照组基础上,联合左炔诺孕酮宫内缓释系统治疗。采用视觉模拟评分法(visual analog score,VAS)评估两组患者痛经疼痛程度,采用彩色多普勒超声检测仪检测治疗前后子宫体积、病灶体积。检测两组治疗前后CA 125、EMAb、HGB、雌二醇(estradiol,E_2)、卵泡刺激素(follicle stimulating hormone,FSH)、促黄体生成素(luteinizing hormone,LH)的水平。结果观察组总有效率高于对照组,差异有统计学意义(P<0.05);两组治疗后子宫体积、病灶体积、VAS评分、CA 125、EMAb均降低,HGB升高,观察组更优于对照组,差异均有统计学意义(P<0.05);两组治疗前后E_2、FSH、LH水平无明显改变(P>0.05)。结论高强度聚焦超声联合左炔诺孕酮宫内缓释系统对子宫腺肌病的疗效确切,能降低患者血清CA 125、EMAb水平,提高HGB水平,保护卵巢功能。
Objective To investigate the effects of high intensity focused ultrasound(HIFU)combined with levonorgestrel on the serum CA125, EMAb, hemoglobin(HGB) and ovarian function in patients with adenomyosis. Methods 90 patients with adenomyosis admitted to Weihai Maternal and Child Health Hospital from January 2015 to October 2017 were randomly divided into observation group and control group, with 45 cases in each group. The control group was treated with HIFU, the observation group was treated with levonorgestrel intrauterine sustained-release system on the basis of the control group. Visual analog score(VAS) was used to evaluate the degree of dysmenorrhea pain in the two groups. Color Doppler ultrasound was used to detect the uterus volume and lesion volume before and after treatment.The levels of CA125, EMAb, HGB, E2, FSH and LH were measured before and after treatment in two groups. Results The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant(P<0.05). After treatment, the uterus volume, lesion volume, VAS score, CA 125, EMAb decreased, HGB increased, and the observation group was better than the control group, the differences were statistically significant(P<0.05). There was no significant change in E2, FSH and LH levels before and after treatment(P>0.05).Conclusion HIFU combined with levonorgestrel intrauterine sustained-release system is effective in adenomyosis, which can reduce serum CA 125 and EMAb levels, increase HGB levels, and protect ovarian function.
