摘要
目的探究胰石蛋白(PSP)对体外循环心脏手术后患者早期感染的预测价值。方法选取需行体外循环心脏手术患者269例为研究对象,记录患者基线资料和术中、术后指标。根据患者出院前是否感染分为感染组和对照组。对比两组基线资料差异,运用重复测量方差分析对比各生化指标含量在不同时间点下的变化。Pearson相关性分析及多元线性回归分析探究各指标间的联系;同时,绘制ROC曲线探究各指标对早期感染的评估能力。结果两组术中及术后指标存在差异(P <0.05);重复测量方差分析提示感染组患者PSP、降钙素原(PCT)的整体含量高于对照组(P <0.05);多元线性回归分析结果显示术后PSP的升高与体外循环时间、APACHEⅡ评分、主动脉阻断时间、呼吸机使用时间、插管留置时间相关(P <0.05)。ROC曲线下,ΔPSP的预测价值略高于ΔPCT。结论 PSP可早期预测体外循环心脏手术后患者早期感染的发生,具有潜在临床应用价值。
Objective To investigate the predictive value of pancreatic stone protein(PSP)on early postoperative infection.Methods 269 patients undergoing cardiopulmonary bypass were selected as the subjects of this study and their baseline data,intraoperative and postoperative indexes were recorded.According to whether the patient was infected before discharge,they were divided into infection group and control group.The difference of baseline data and other detection indexes between the two groups were compared.Repeated measurement analysis of variance was used to analyze difference in the content of biochemical indicators at different time points.Pearson correlation analysis and multiple linear regression analysis(stepwise method)were applied to explore the relationship between the indicators.Meanwhile,the ability of each potential indicator to assess early infection in patients undergoing cardiopulmonary bypass cardiac surgery was calculated by ROC curve.Results There were differences between the two groups in the intraoperative and postoperative indexes(P<0.05).Repeated measures analysis of variance suggested that the overall level of PSP and PCT in infection group was higher than that of control group(P<0.05).The multiple linear regression analysis showed that the increase of postoperative PSP was associated with cardiopulmonary bypass time,A PACHE II score,aortic blockade time,ventilator use time,and indwelling catheter intubation time(P<0.05).The predictive value ofΔPSP was slightly higher thanΔPCT under the ROC curve.Conclusion PSP can predict the occurrence of infection early,which has potential clinical appli cation value.
