血小板膜糖蛋白Ⅱb、Ⅲa基因多态性与急性缺血性脑卒中的关系研究 被引量:9

Relationship between Gene Polymorphism of Platelet Membrane Glycoprotein Ⅱb,Ⅲa and Acute Ischemic Stroke

摘要 背景血小板膜糖蛋白(GP)Ⅱb/Ⅲa受体与纤维蛋白原有效结合是血小板聚集、血栓形成的最后通路,近年来从分子生物学角度研究GP基因多态性与急性缺血性脑卒中(AIS)的关系成为研究热点。目的分析GPⅡb、Ⅲa基因多态性与AIS的关系。方法选取2017年10月—2018年11月陕西省人民医院神经内一科收治的AIS患者110例作为病例组,另选取同期在门诊体检中心的体检健康者90例作为对照组。比较两组受试者一般资料(包括性别、年龄及吸烟、饮酒情况)、实验室检查指标〔包括PAC-1阳性血小板百分比、总胆固醇(TC)、三酰甘油(TG)、空腹血糖(FPG)及同型半胱氨酸(Hcy)〕及GPⅡb、Ⅲa基因型和等位基因分布频率,并比较不同GPⅡb、Ⅲa基因型患者PAC-1阳性血小板百分比。结果 (1)两组受试者男性比例、年龄、吸烟率、饮酒率及TG比较,差异无统计学意义(P>0.05);病例组患者PAC-1阳性血小板百分比、TC、FPG、Hcy高于对照组(P<0.05)。(2)病例组与对照组受试者基因型分布频率均符合Hardy-Weinberg平衡定律(P>0.05)。病例组患者GPⅡb ab基因型、bb基因型及b等位基因分布频率高于对照组(P<0.05);两组受试者GPⅢa基因型和等位基因分布频率比较,差异无统计学意义(P>0.05)。(3)GPⅡb bb基因型患者PAC-1阳性血小板百分比高于aa基因型(P<0.05)。(4)不同GPⅢa基因型患者PAC-1阳性血小板百分比比较,差异无统计学意义(P>0.05)。结论 GPⅡb基因多态性与AIS的发生有关,携带b等位基因者可能通过增强血小板活化程度而增加AIS发生风险,而GPⅢa基因多态性则与AIS的发生无关。 Background Effective combination of platelet membrane glycoprotein(GP) Ⅱ b/Ⅲ a receptor and fibrinogen is the final pathway of platelet aggregation and thrombosis,thus relationship between gene polymorphism of GP and acute ischemic stroke(AIS)became the research hotspot in recent years. Objective To analyze the relationship between gene polymorphism of GP Ⅱ b,Ⅲ a and AIS. Methods From October 2017 to November 2018,110 patients with AIS were selected as case group in the First Department of Neurology,Shaanxi Provincial People’s Hospital,meanwhile90 healthy volunteers admitted to this hospital for physical examination were selected as control group. General information(including gender,age,smoking rate and drinking rate),laboratory examination results(including PAC-1 positive platelet percentage,TC,TG,FPG and Hcy),genotypes and allele distribution frequency of GP Ⅱ b,Ⅲ a were compared between the two groups,moreover PAC-1 positive platelet percentage was compared in patients with different genotypes of GP Ⅱ b,Ⅲ a.Results(1)No statistically significant difference of male ratio,age,smoking rate,drinking rate or TG was found between the two groups(P>0.05),while PAC-1 positive platelet percentage,TC,FPG and Hcy in case group were statistically significantly higher than those in control group(P<0.05).(2)Genotype distribution frequency in both case group and control group was accord with Hardy-Weinberg equilibrium(P>0.05). Distribution frequency of ab genotype,bb genotype and b allele of GP Ⅱ b in case group were statistically significantly higher than those in control group,respectively(P<0.05);while no statistically significant difference of genotypes or allele distribution frequency of GP Ⅲ a was found between the two groups(P>0.05).(3)PAC-1 positive platelet percentage in patients with bb genotype of GP Ⅱ b was statistically significantly higher than that in patients with aa genotype(P<0.05).(4)No statistically significant difference of PAC-1 positive platelet percentage was found in patients with different genotypes of GP Ⅲ a(P>0.05). Conclusion Gene polymorphism of GP Ⅱ b is significantly correlated with the occurrence of AIS,b allele may increase the risk of AIS through strengthening the degree of platelet activation,while gene polymorphism of GP Ⅲ a is not significantly correlated with the occurrence of AIS.
